ارتباط چگالی انرژی رژیم غذایی با اختلالات قلبی-متابولیکی در زنان تهرانی

<p align="justify"><strong>مقدمه و هدف</strong>: مطالعه ی حاضر با هدف ارزیابی ارتباط میان چگالی انرژی رژیم غذایی و شیوع عوامل خطر کاردیومتابولیک در زنان تهرانی صورت گرفت. <strong>روش کار</strong>: در این مطالعه مقطعی دریافت های غذایی معمول 486 زن تهرانی با استفاده از پرسشنامه ی بسامد خوراک اعتبارسنجی شده ارزیابی شد. چگالی انرژی رژیم غذایی با تقسیم دریافت انرژی روزانه گزارش شده (کیلوکالری در روز) تقسیم بر وزن کل غذای مصرفی (به استثنای نوشیدنی ها) بدست آمد. سطح قند خون ناشتا، پروفایل های لیپیدی و فشار خون نیز اندازه گیری شد. وجود حداقل یک عامل خطر و حداقل دو عامل خطر از سه عامل خطر اصلی بیماری های قلبی عروقی (پرفشاری خون،دیس لیپیدمی،دیابت) ارزیابی شد. برای تعیین ارتباط بین چگالی انرژی رژیم غذایی و عوامل خطر کاردیومتابولیک نسبت شیوع در مدل های مختلف با توجه به مخدوش گرها بدست آمد.</p>
<p align="justify"><strong>نتایج</strong>: میانگین چگالی انرژی رژیم غذایی 0.35±1.77 کیلوکالری در گرم بود. مصرف رژیم های غذایی با چگالی بالای انرژی در ارتباط با دریافت های بالاتر انرژی،چربی رژیم غذایی، کلسترول، روغن های گیاهی، غلات تصفیه شده و محصولات لبنی پرچرب و دریافت های کمتر کربوهیدرات، میوه ها، سبزیجات، گوشت و ماهی بود. همچنین رژیم غذایی با چگالی بالای انرژی در ارتباط با سطوح بالاتر تری گلیسرید سرم، کلسترول تام و LDL کلسترول و سطوح پایین تر HDL کلسترول بود. زنانی که در بالاترین چارک چگالی انرژی رژیم غذایی بودند در مقایسه با آنهایی که در پایین ترین چارک قرار داشتند شانس بیشتری برای ابتلا به دیس لیپیدمی (%61 در مقابل 31%، P<0.05) ،حداقل یک عامل خطر متابولیک (%68 در مقابل 35% ، P<0.05) و حداقل دو عامل خطر کاردیومتابولیک (%29 در مقابل 10%، P<0.05) داشتند. کنترل بیشتر برای شاخص توده ی بدنی اندکی این ارتباطات را ضعیف تر کرد. در مجموع هیچ ارتباطی بین مصرف رژیم غذایی با چگالی بالای انرژی و خطر داشتن دیابت یا فشار خون چه قبل و چه بعد از تعدیل مخدوش گرها دیده نشد.</p>
<p align="justify"><strong>نتیجه نهائی</strong>: مصرف رژیم غذایی با چگالی بالای انرژی به طور چشمگیری با شیوع دیس لیپیدمی، حداقل یک و حداقل دو عامل خطر کاردیومتابولیک در میان زنان تهرانی همراه بود. مطالعات آینده نگر بیشتری نیاز است تا این یافته ها را تایید کنند.</p>
<p align="justify"><strong>Introduction & Objective</strong>: This cross-sectional study was undertaken to assess the association between dietary energy density and prevalence of the cardiometabolic risk factors among Te-hranian adult women.</p>
<p align="justify"><strong>Materials & Methods</strong>: In this cross-sectional study we assessed habitual dietary intakes of 486 Tehranian adult women by the use of a validated food frequency questionnaire. Dietary energy density (DED) was calculated as each individual’s reported daily energy intake (kcal/d) into total weight of foods (excluding beverages) consumed (g/d). Fasting plasma glucose (FPG), lipid profiles and blood pressure were measured. Diabetes (FPG? 126 mg/dL), dyslipidemia (based on Adult Treatment Panel III) and hypertension (based on Joint National Committee VI) were defined. The presence of “at least one risk factor” and "at least two risk factors" of the three major risk factors for cardiovascular disease (hypertension, dyslipidemia and diabetes) were also evaluated. To explore the associations between DED and cardio-metabolic risk factors, we obtained prevalence ratios in different models accounting for confounders.</p>
<p align="justify"><strong>Results</strong>: Mean dietary energy density was 1.77±0.35 kcal/g. Consumption of energy-dense diets was associated with higher intakes of energy, dietary fat, cholesterol, vegetable oils, refined grains and high-fat dairy products and lower intakes of dietary carbohydrates, fruits, vegetables , meat and fish. Adherence to an energy-dense diet was associated with elevated levels of serum triglycerides, total- and LDL-cholesterol and lower levels of serum HDL-cholesterol. Women in the top quartile of DED were more likely to have dyslipidemia (61% vs. 31%, P<0.05), at least one (68% vs. 35%, P<0.05) and at least two (29% vs. 10%, P<0.05) cardiometabolic risk factors as compared with those in the bottom quartile. Additional control for BMI slightly attenuated the associations. No overall significant associations were found between consumption of energy-dense diets and risk of having diabetes or hypertension either before or after adjustment for confounders. <strong>Conclusion</strong>: Consumption of energy-dense diets was significantly related to the high prevalence of dyslipidemia, at least one and at least two risk factors among Tehranian adult women. Prospective studies are required to confirm our findings.</p>
