<p align="justify">
<strong>مقدمه وهدف</strong>: ثبت اطلاعات پزشکی در پرونده های بیمارستانی (Hospital Records) در حقیقت مستند سازی فعالیت های تیم پزشکی در بیمارستان است. با توجه به اهمیت مسئله ، مطالعه حاضر با هدف تعیین نواقص پرونده نویسی در بیمارستان های آموزشی دانشگاه علوم پزشکی همدان در سال 88 انجام شد.
</p><p align="justify"><strong>روش کار</strong>: در این مطالعه توصیفی مقطعی ، اطلاعات پرونده های آموزشی بیمارستانهای دانشگاهی شهر همدان استخراج ودر چک لیست وارد گردید. نمونه گیری به شیوه نمونه گیری در دسترس از تمامی گروههای آموزشی انجام گردید. چک لیست تحقیق شامل16 سوال در خصوص وجود دستور بستری و ترخیص، وجود خلاصه پرونده ، ثبت تشخیص نهائی، ثبت اقدامات درمانی و...... بود که در هر مورد با پاسخ بلی یا خیر ثبت می شد. اطلاعات حاصل با نرم افزار spss16 آنالیز گردید.
</p><p align="justify"><strong>نتایج</strong>: پس از بررسی 457 پرونده بیمارستانی از 4 بیمارستان آموزشی این نتایج بدست آمد: 94% پرونده ها دارای دستور بستری بودند .ثبت تاریخ، مهر وامضاء پزشک در 93 درصد پرونده ها وجود داشت.که این رقم با فراوانی ثبت تاریخ ، مهر وامضا توسط کارورزان ودستیاران برابر بود و به همین نسبت نیز فرم شرح حال در پرونده های آموزشی وجود داشت. 88 درصد از فرم های شرح حال پرونده ها توسط کارورز و یا دستیارمهرو امضا شده بودند. تشخیص های افتراقی در 75 درصد، ثبت تشخیص نهائی در 90 درصد و دستور ترخیص توسط پزشک در 84 در صدپرونده ها وجود داشت. 86 درصد پزشکان اقدامات درمانی و یا اعمال جراحی را در پرونده ثبت کرده بودند .ثبت اقدامات پاراکلینیکی در 83 درصد پرونده ها انجام شده بود. تنها 61 درصد از پرونده ها در برگه سیر بیماری امضا ومهر دستیار و یا کارورز را داشتند. برگه های مراقبت پرستاری دراکثریت قریب به اتفاق پرونده ها مهر و امضا شده بود (99درصد). در پرونده های آموزشی نیازمند مشاوره ، در 83 درصد اطلاعات دقیق مشاوره از قبیل ساعت درخواست، تاریخ و... ثبت شده بود.همینطور در 82 درصد موارد مشاوره، مهر وامضا پزشک مشاور درج شده بود.
</p><p align="justify"><strong>نتیجه نهائی</strong>: یافته های پژوهش نشان داد که پرونده های بیمارستانی در ثبت اطلاعات دارای نواقص جدی می باشند. به نظر میرسد علل عدم رعایت اصول صحیح پرونده نویسی در بیمارستان های آموزشی چند گانه باشد که ازجمله آنها می توان به شلوغی وازدحام زیاد بیماران ، عدم احساس مسئولیت توسط دانشجویان وکارورزان و مهمتر ازهمه عدم آموزش کافی در این خصوص اشاره کرد.

<strong>Introduction & Objectives</strong>: Hospital records are representative evidences of medical team activities. In this study, we analyzed hospital records in Hamadan teaching hospitals to find out the problem extent and possible solutions for the problem.</p><p align="justify">
<strong>Materials & Methods</strong>: In a cross-sectional study, hospital records from teaching hospitals were gathered and put in check lists. We used convenient sampling from all departments, so that by referring to hospital achieve, all new discharged cases from different wards were extracted. We used a 16 item check list which targeted some basic questions like: admission order, discharge order, early and final diagnosis and so on. In each case perfect answer was registered in yes or no boxes. Collected data were analyzed by SPSS16 hardware.
</p><p align="justify"><strong>Results</strong>: We achieved the following results after analyzing 457 records from 4 teaching hospitals there were admission note in 94% of the patients' files. 93% of physicians and residents had signed the orders. 88% of the history sheets were being singed by medical students and/or residents. Differential diagnoses were present in only 75% of cases. Final diagnoses were found in 90% and discharge notes in 84% of the files. 86% of physicians had recorded therapeutic and/or surgical procedures. Paraclinical procedure recordings were present in 83% of the files. Only 63% of residents and/or interns had signed their progress notes. And nursing papers were signed in 99% of records. There was exact counseling information in 83% of the files which needed to be consulted meanwhile 82% of the consulted files had been signed by physicians.
</p><p align="justify"><strong>Conclusion</strong>: This study shows that, there are important defects in hospital records. It seems that there are multiple factors contributing to the problem, such as overcrowding of the hospitals, careless medical students and the most important factors is insufficient training about the problem.
