<p align="justify">
<strong>مقدمه</strong>: پارگی رحم در حاملگی نادر است ولی می تواند در اغلب اوقات با عوارض فاجعه آمیزی همراه باشد و با شیوع بالای مورتالیتی و موربیدیتی مادر و جنین همراه گردد. شایع ترین علت پارگی رحم جدا شدن اسکار انسزیون قبلی سزارین می باشد. سایر عوامل زمینه ساز شایع پارگی رحم شامل دستکاریها یا اعمال تروماتیزه کننده قبلی مانند کورتاژ، سوراخ شدگی رحم یا میومکتومی و یا تحریک بیش از حد یا نامناسب رحم با اکسی توسین می باشد.
</p><p align="justify"><strong>معرفی بیمار</strong>: در این گزارش، یک مورد پارگی خودبخود رحم در زن 36 ساله G5P2Ab2L1 با حاملگی دوقلویی در سن حاملگی 26-25 هفته معرفی می شود. بیمار به علت بررسی پره کلامپسی در بخش بستری می شود که در سابقه بیمار یک میومکتومی به طریق لاپاروسکوپی وجود دارد که 3 ماه بعد از آن به علت نازایی ثانویه تحت IVF قرار می گیرد. در طی بستری به صورت ناگهانی دچار خونریزی واژینال مختصر و عدم حرکت جنین ها می شود که به دنبال سونوگرافی اورژانسی، گزارش دو جنین مرده با کاهش شدید مایع آمنیوتیک و میوم های رحمی با بزرگترین سایز 90 میلی متر داده می شود.
</p><p align="justify" style="direction: rtl"><strong>نتیجه نهایی</strong>: جهت بیمار سعی در زایمان واژینال می شود که به علت عدم پاسخ جهت سزارین آماده می گردد حین عمل ، پارگی رحم در محل قبلی میومکتومی به همراه دو جنین مرده داخل شکم وجود داشت در ضمن میومی در بافت رحم وجود نداشت و میوم های گزارش شده توسط سونوگرافی لخته های خون و امنتوم بود . محل پارگی ترمیم شد.

<strong>Introduction</strong>: Uterine rupture in pregnancy is rare and often catastrophic with high incidence of fetal and maternal morbidity and mortality. The most common cause of uterine rupture is separation of previous cesarean hysterotomy scar. Other common predisposing factors to uterine rupture are previous traumatizing operations or manipulations such as curettage, perforation, myomectomy and excessive or inappropriate uterine stimulation with oxytocin.
</p><p align="justify"><strong>Case Report</strong>: In this case, spontaneous rupture of uterine at 25-26 weeks gestation (twin pregnancy) in a 36 year old woman has been reported(G5P2A2L1). She was hospitalized so that preeclampsia can be rule out for her. She underwent laparoscopic removal of myoma and 3 months later IVF was done for her because of secondary infertility. During hospitalization, suddenly she had mild vaginal bleeding and decreased fetal movement. Emergency sonography reported that the two fetuses died and severe oligohydramnius and myomatosis uterine with the biggest size was 90mm.
</p><p align="justify"><strong>Conclusion</strong>: Try of labor was done for her but she didn't respond to induction and the patient was qualified for cesarean section. During the operation, the rupture of the uterine wall was seen in the site of the previous myomectomy and the two dead fetuses were observed in the abdomen without myoma in the uterin. The myoma that was reported by the sonologist was hematoma and omentoma. Uterine wall was repaired
