<p align="justify">
<strong>مقدمه و هدف</strong>: از دست دادن سد دفاعی پوستی، سرکوب ایمنی، تعلل در ارجاع بیمار به بیمارستان ، طول اقامت در بیمارستان و اقدامات عملی ـ درمانی عواملی هستند که جملگی باعث می شوند افراد دچار سوختگی در اغلب موارد در معرض خطر به عفونت های متعدد گردند، لذا بررسی علل عفونت های باکتریال در بیماران دچار سوختگی در مرکز سوختگی اصفهان هدف این مطالعه می باشد.
</p><p align="justify">
<strong>روش کار</strong>: این مطالعه به صورت توصیفی ـ مقطعی و مدت اجرا یک سال در مرکز سوانح و سوختگی اصفهان از اول شهریور ماه سال 1383 الی اول مهر ماه سال 1384 انجام گرفت. تعداد 106 بیمار واجد شرایط که در طی دو روز اول پس از بستری شدن در مرکز فوق فاقد علائم و نشانه های بروز عفونت بوده، طبق تعریف مرکز کنترل بیماریها در آمریکا (CDC) وارد مطالعه گردیدند روزانه معاینه بالینی و بررسی وضعیت بیماران و مطالعه پرونده های آنان جهت یافتن علائم و نشانه های عفونتهای بیمارستانی و کسب اطلاعات مربوطه از پرسنل بیمارستان انجام گرفت. کشت سواب سطحی (Culture swab) ، کشت خون در موقع تب و کشت بافت (Tissue culture) از نمونه بیوپسی زخم سوختگی و کشت ادرار در طول مدت بستری بیمار از واجدین شرایط ورود به مطالعه به عمل آمد. داده ها به وسیله نرم افزار 10 SPSS و با استفاده از آزمون مجذور کای (Chi-square) ، و ضریب کاپا (Kappa coefficient) تجزیه و تحلیل شدند و مقادیر P کمتر از 0.05 معنی دار در نظر گرفته شد.</p><p align="justify">
<strong>نتایج</strong>: از تعداد 106 بیمار واجد شرایط در این مطالعه 91 نفر (85.8%) حداقل دچار یک نوع عفونت شدند که عفونتهای ادراری، خونی و زخم سوختگی به ترتیب 28 (26.4%)، 30 (28.3%) و 91 (85.8%) بود. در طی این بررسی کشت خون و کشت سوآب سطحی زخم نشان داد که پسودوموناس آئروژینوزا به ترتیب 27.4% و 54.4% شایعترین ارگانیسم در بین عوامل عفونی زخم سوختگی بود. همچنین کشت ادرار در این بیماران نشانگر آن است که اشریشیای کلی به عنوان ارگانیسم اصلی 4/6% ایجاد کننده عفونت ادراری بوده است.

<strong>نتیجه نهائی</strong>: بیوپسی از زخم سوختگی آن هم به صورت یک روز در میان و بررسی هیستوپاتولوژیک نمونه و بسته به شرایط کشت خون و کشت ادرار جملگی می توانند جهت شناسایی زودرس عفونت زخم سوختگی کمک مؤثری نمایند.

<strong>Introduction & Objective</strong>: Burn patients are at risk of acquiring infection because of destroy skin barrier, suppression of immunity, prolonged hospitalization, and invasive therapeutic and diagnostic procedure, risk of acquiring infection is high among burned patients. The aim of this study was to assess the incidence rate of bacterial etiology and infections among burn patients in the burn center of Isfahan.
<strong>
Materials & Methods</strong>: The study was conducted descriptive – cross sectional during a period of one year, (from august 2004 until September 2005). A total of 106 patients presenting with no signs and symptoms of infection within the first 48 hours of admission were included. CDC definition for nosocomial infections was applied. Each patient’s clinical examinations and records investigated daily. Swab culture, blood culture (during fever time), tissue culture from biopsy specimen of burn wound and urine culture obtained. The data were analyzed and interpreted using SPSS 10 Software, using Chi – square and Kappa Coefficient. P.value < 0.05 was significant.

<strong>Results</strong>: One-hundred and six patients met the inclusion criteria, 91 (85/8%) acquired at least one type of infection, including, urinary tract 28 (26/4%), blood stream 30 (28/3%), and burn wound 91(85/8%). Pseudomonas aeroginosa was the most common causative agent isolated from blood culture and swab culture, 27/42% and 54/4% respectively. Also, E.coli was the major casautive agent of urinary tract infections (6.4%) isolated from urine culture of these burn patients.
</p><p align="justify">
<strong>Conclusion</strong>: The results indicated that, biopsy from burn wounds and study of histopathologic specimen day by other day depends on blood and urine culture conditions overall can be effective for early detection of burn wounds infections.
