fa تخمین وزن جنین با استفاده از اندازه ارتفاع رحم و دور شکم مادر The Estimation of Fetal Weight with Mother Fundal Height and Abdominal Girth سایر تخصص هاي باليني Other Clinical Specialties پژوهشي Original <span id="ctl00_ContentPlaceHolder1_DataList1_ctl00_abstractLabel" style="font: 12px/15pt tahoma; color: rgb(45, 135, 179); text-transform: none; text-indent: 0px; letter-spacing: normal; word-spacing: 0px; white-space: normal; widows: 1; font-size-adjust: none; font-stretch: normal; background-color: rgb(247, 246, 243); -webkit-text-stroke-width: 0px;">تخمین وزن جنین قبل از زایمان از اهمیت خاصی برخوردار است زیرا جنینهای بزرگتر و کوچکتر از حد طبیعی ممکن است در هنگام زایمان دچار مشکلات متعددی شوند .  پژوهش حاضربا هدف تعیین وزن جنین با استفاده از اندازه ارتفاع رحم ودورشکم مادردر مراجعین جهت زایمان به بیمارستان فاطمیه همدان درسال 1379 اجرا گردید. <span id="ctl00_ContentPlaceHolder1_DataList1_ctl00_abstractLabel" style="font: 12px/15pt tahoma; color: rgb(45, 135, 179); text-transform: none; text-indent: 0px; letter-spacing: normal; word-spacing: 0px; white-space: normal; widows: 1; font-size-adjust: none; font-stretch: normal; background-color: rgb(247, 246, 243); -webkit-text-stroke-width: 0px;">          طی یک بررسی توصیفی از طریق نمونه گیری تصادفی تعداد200 نفر ( 100 نفر اول زا , 100نفر چندزا ) از زنان مراجعه کننده بر اساس شرایط مورد نظرکه حاملگی نرمال و یک قلو , وزن کمتر از 90 کیلوگرم در زمان مراجعه , مثانه تخلیه شده و منفی بودن جایگاه عضو نمایش جنین بوده است بعنوان نمونه انتخاب شدند و متغیرهای لازم که شامل اندازه ارتفاع رحم و دور شکم مادر (بر حسب سانتی متر) , وزن تخمینی جنین با استفاده از حاصلضرب دو عدد بدست آمده از اندازه گیریهای فوق و وزن واقعی بلافاصله پس از تولد نوزاد می باشد مورد بررسی قرار گرفته است . <span id="ctl00_ContentPlaceHolder1_DataList1_ctl00_abstractLabel" style="font: 12px/15pt tahoma; color: rgb(45, 135, 179); text-transform: none; text-indent: 0px; letter-spacing: normal; word-spacing: 0px; white-space: normal; widows: 1; font-size-adjust: none; font-stretch: normal; background-color: rgb(247, 246, 243); -webkit-text-stroke-width: 0px;">          نتایج بررسی واحدهای پژوهش نشان داد که میانگین اندازه ارتفاع رحم و دور شکم مادر بترتیب 33.92±2.85 و 97±6.2 سانتی متر , میانگین وزن تخمینی جنین با استفاده از حاصلضرب دو عدد فوق 3088±382 گرم و میانگین وزن واقعی بلافاصله پس از تولد جنین 3060±344 گرم بر آورد شده است . میزان همبستگی بین وزن تخمینی و وزن واقعی بلافاصله پس از تولد جنین 0.61 محاسبه گردید. حساسیت و ویژگی حاصلضرب اندازه ارتفاع رحم و دور شکم مادر در تشخیص جنینهای بزرگتر از اندازه طبیعی بترتیب 83.3% و 86.5% و در تشخیص جنینهای کوچکتر از اندازه طبیعی بترتیب 48.4% و 96.2% می باشد . <span id="ctl00_ContentPlaceHolder1_DataList1_ctl00_abstractLabel" style="font: 12px/15pt tahoma; color: rgb(45, 135, 179); text-transform: none; text-indent: 0px; letter-spacing: normal; word-spacing: 0px; white-space: normal; widows: 1; font-size-adjust: none; font-stretch: normal; background-color: rgb(247, 246, 243); -webkit-text-stroke-width: 0px;">          نتایج بدست آمده بیانگر اثر مثبت اندازه گیری ارتفاع رحم و دور شکم مادر در تخمین وزن جنین در زمان تولد است.  Considering the hazards of macrosomia and low birth weight, the estimation of birth weight before delivery is of paramount importance. This study was performed to estimate the fetal weight on the base of mother fundal height and abdominal girth in women referred to Fathemieh hospital in Hamadan for delivery in 2000.           We conducted a descriptive study with 200 women (100 primipara and 100 multipara) who were selected through a random sampling. Inclusion criteria weresingle normal pregnancy, body weight less than 90 kg, empty bladder, negative station of presentation, intact amniotic bag. The variables included the uterine height and abdominal girth (in Cms). Estimated fetal weight obtained through multiplying the two values of above variables and    then comparing with the weight of newborn immediately after delivery.              The results showed that the mean of mother fundal height and abdominal girth were 33.92±2.85 cm and 97±6.2 cm respectively. The mean of estimated fetal weight and measured weight were 3088±382g and 3060 ±344g respectively.
