بررسی ارتباط بین حساسیت صوتی سیستم دهلیزی و توانایی تشدید رفلکس عضلانی برانگیخته صوتی گوش میانی در بزرگسالان دارای شنوایی طبیعی

<strong>مقدمه و هدف</strong>: سیستم دهلیزی به صدا حساس است و بیشترین حساسیت صوتی متعلق به ساکول است.آوران های دهلیزی تا عضلات گوش میانی (از قبیل عضله رکابی) امتداد دارند. هدف این مطالعه بررسی ارتباط بین شنوایی دهلیزی با رفلکس عضلانی برانگیخته ی صوتی گوش میانی با محرک 500 هرتز بود. </p><p align="justify"><strong>روش کار</strong>: این مطالعه از نوع مقطعی - مقایسه ای و محل آن بخش شنوایی شناسی کلینیک تخصصی وفوق تخصصی شیخ الرئیس همدان بود. گروه مطالعه را 30 فرد مبتلا به حس سرگیجه و 30 فرد سالم تشکیل می دادند. معیار ورود به مطالعه، طبیعی بودن عملکرد شنوایی افراد براساس آزمون های ادیومتری صوت خالص، ایمیتانس آکوستیک و امتیاز بازشناسی گفتار بود. براساس آزمون رفلکس صوتی مسیر همان طرفی با محرک 500 هرتز به دو گروه طبیعی وغیر طبیعی تقسیم بندی شدند، سپس تحت ارزیابی آزمون cVEMPs قرار گرفتند و در خاتمه به سه گروه N، CVUA و IVA طبقه بندی شدند. </p><p align="justify"><strong>نتایج</strong>: سی گوش مبتلا (IVA) با کاهش حساسیت تحریک پذیری سیستم دهلیزی که نتایج غیرطبیعی در آزمون cVEMPs داشتند، در ارزیابی رفلکس صوتی با صوت خالص 500 هرتز نیز غیرطبیعی بودند. در صورتیکه گوش های غیر مبتلا (CVUA) و طبیعی (N) نتایج طبیعی داشتند. مقایسه ی میانگین های cVEMPs (p13 و (n23و رفلکس صوتی با صوت خالص 500 هرتز در بین سه گروه معنی دار بود. ارتباط بین آستانه ی رفلکس صوتی با صوت خالص 500 هرتز و زمان تاخیر p13 معنی دار بود. ارتباط معنی داری بین زمان تاخیر n23 و آستانه ی رفلکس صوتی با صوت خالص 500 هرتز مشاهده گردید. </p><p align="justify"><strong>نتیجه نهایی</strong>: تحریک صوتی سیستم دهلیزی در بزرگسالان با شنوایی طبیعی، توانایی ایجاد رفلکس عضلانی برانگیخته ی صوتی گوش میانی با محرک 500 هرتز را تشدید می نماید.

<strong>Introduction & Objective</strong>: The vestibular system is sound sensitive and the sensitivity is related to the saccule. The vestibular afferents are projected to the middle ear muscles (such as the stapedius). The goal of this research was studying the relationship between the vestibular hearing and the sound-evoked muscle reflex of the middle ear to 500 HZ. </p><p align="justify"><strong>Materials & Methods</strong>: This study was a cross sectional-comparison done in audiology department of Sheikholreis Clinic (Hamadan, Iran). The study groups consisted of thirty healthy people and thirty patients with benign paroxysmal positional vertigo. Inclusion criteria of the present study were to have normal hearing on pure tone audiometry, acoustic reflex, and speech discrimination scores. Based on ipsilateral acoustic reflex test at 500HZ, they were divided to normal and abnormal groups. Then they were evaluated by cervical vestibular evoked myogenic potentials (cVEMPs) and finally classified in three groups (N) Normal ear ,(CVUA) Contra lateral vertiginous ear with unaffected saccular sensitivity to sound,(IVA) Ipsilateral vertiginous ear with affected saccular sensitivity to sound. </p><p align="justify"><strong>Results</strong>: Thirty affected ears (IVA) with decreased vestibular excitability as detected by ab-normal cVEMPs, revealed abnormal findings of acoustic reflex at 500HZ. Whereas, both un-affected (CVUA) and normal ears (N) had normal results. Multiple comparisons of mean values of cVEMPs (p13,n23) and acoustic reflex at500HZ among the three groups were sig-nificant. The correlation between acoustic reflex at 500HZ and p13 latencies was significant. The n23 latencies showed significant correlation with acoustic reflex at 500HZ. </p><p align="justify"><strong>Conclusion</strong>: The vestibular sensitivity to sound retains the ability to trigger sound-evoked re-flex of the middle ear at 500 HZ.
