Study of 450 Solitary Thyroid Nodules in Hamadan

The prevalence of nodular thyroid disease increases with aging, especially during sixth decade of the life, 5-10% of the general population have one or more palpable thyroid nodules. Nodular goiter is more common in persons living in areas with iodine deficiency. Cold thyroid nodules have been estimated 2.5 times more frequent in iodine areas in comparison with the iodine sufficient areas.
The evaluation and management of solitary thyroid nodules constitute a very common problem in medicine.
The concern with thyroid nodules is that they may be malignant, the challenge, that only a small minority are in fact malignant.
The prevalence of nodular goiter is noticeable in Hamadan (hyperendemic area). We studied the clinical, radio isotopic, ultrasonographic and pathologic characteristics of 450 solitary thyroid nodules in Hamadan.
59% of patients were female and 41% were male. 22% nodules were systic & 78% were solid. 30% of solid nodules were warm & 41% were cold in radioisotopic study. About 11% of cold nodules were malignant and papillary cell carcinoma were the most common type of thyroid malignancy.

Transforming Growth Factor-alpha Induce Synthesis of alpha-1-Antitryspin in Human Hepatoma Cell Line (HepG2)

Cytokines protein or glycoproteins synthesized by a wide range of cell types including lymphocytes, macrophages, platelets, and monocytes. Cytokines induse synthesis of acute phase protein in hepatocytes. So far eight cytokines has been shown can induce acute phase protein synthesis in hepatocyte and hepatoma cell culture. There are evidences that transforming growth factor-alpha (TGF-a) secrets by tumor cells. In the other hand conditioned medium from ovarian cancer cells can stimulate API synthesis in hepatoma cell line. To show the effect of TGA-a on API synthesis in human hepatoma cell line HepG2, two different concentration of TGF-a were added to culture media of HepG2 cells. API concentration culture media was measured using rocket immunoassay. The results showed TGF-a in 5 ng/ml increased synthesis of API abut 29%. In higher concentration, TGF-a showed lower effect. It can be suggested that TGF-a plays role in preventing the effect of proteolytic enzymes by inducing synthesis of anti-proteases.

The Study of Obesity and Leanness in Primary – School Children, Kerman, Iran

The status of obesity leanness is unknown among Iranian primary-school age children. We measured weight, height, arm, chest, waist, hip and thigh among 1250 boys and 1250 girls aged 6-12 years in Kemanian primary-school to evaluate the status of obesity and leanness. The criteria for leanness, underweight, desirable weight, overweight and obesity used in the present study were percentiles of body mass index (BMI) (kg/m2) : <5th, 5-15th, 15-85th and 85-95th and > 95th respectively. The range of BMI for desirable weight in boys and girls were 13.7-16.5 and 13.4-16.8 respectively. Range of overweight was 16.5-18.4 for boys and 18.8 for boys and 16.8-18.8 for girls. BMI over 18.4 for boys and 18.8 for girls were considered obesity. The mean (Standard deviation – SD) of BMI for both boys and girls was 15.1 (2.1). the mean (SD) of waist –to – hip and chest – to – hip ratio was 0.85 (0.001) and 0.93 (0.001) respectively in this study.

The Reasons of Not Using Contraception in Unwanted Pregnancies

One of the main objects of family planning is to prevent unwanted pregnancies. It is apparent that this way maternal morality and hospital works will decrease.
The goal of present study was to approach the reason of unwanted pregnancies among mothers who did not use contraception. 933 mothers that delivered in Hamadan province during 1995, were selected randomly and interviewed. 42.1% of their newborns were unwanted, and 25.7% of these mothers used no way contraception method.
The most frequent reasons of not using contraception are listed below:
Advises of medical staff….21.8%
Family desire …………….18.8%
No reason ………………...15.8%
Fear of complication ………7.9%
It seems the improvement of the knowledge of medical staff, young couples and general population can decrease unwanted pregnancies.

The Study of Skin Disease in the Mentally Subnormal

Mental deficiency is not a disease in its own right but a condition resulting from a variety of causes, some inborn and others acquired. The skin abnormalities of mental defectives fall into three broad groups: 1. Cutaneous lesions specifically associated with syndromes of genetic or developmental origin. 2. Non-specific cutaneous lesions showing an increased prevalence in mental defectives. 3. non-specific cutaneous lesions.
We examined 110 mental retarded cases in Fayyaz Bakhsh center during 7 months from April to November 1994. Their skin disease were diagnosed by physical exam. Smear, biopsy and pathology.
Most cases were predominantly children and skin disease in 5-15 years age group was more than the other group.
In patients with IQ < 25 , the incidence of skin disease was more than the others.
Also the incidence of skin disease in mental retarded with anomaly was higher than the other groups.
The infectious skin disease in severe mental retarded was high. Most of skin disease were in second and third group.
The results of statistical comparison showed that impetigo was the most common skin disease in this study.

The extensive use of tricyclic antidepression drugs, the approximation of the therapeutic levels of the drugs to toxic level and the availability of the drugs to depressed patients who have potentially the idea of committing suicide has made these drugs one of the most popular drugs for the patients who commit suicide in a retrospective study on 24 patients in Sina Medical Center in Hamadan. We came to this conclusion that prompt transfer of the patients and gastric lavage the most important way of the management. Concerning the fast absorption of the drug from G.I. tract and the direct relation between E.K.G. changes and plasma level of the drug, we can use the E.K.G. changes as a criterion to estimate the level of plasma augmentation to toxic levels and also we can surely discharge the patients if there is no E.K.G. changes and the patient is quite conscious.

Hypervitaminosis A Induced First Arch Syndrome

In this study we examine the effect(s) of excess vitamin A on developing first branchial arch. Intraperitoneal administration of 10000 u/Kg of vitamin A at zero and 12 hrs, on day of gestation (vaginal plug = zero) induced high incidences of craniofacial defect such as mandibular hypoplasia and cleft palate. It is well known that distruption among the neural crest cells migration and/or differentiation during face formation might cause facial malformation. Based on our results, we may conclude that excess vitamin A probably interfere with crest cells migration in cranifacial as it may induced excess cell death. The later phenomena may also cause facial defects.

Measurement of Non-Enzymatic Glycosylated Serum Proteins (GSP) by Improved Thiobarbitoric Acid Assay

Several classes of protein may undergo non-enzymetic addition of glucose derived product. Hemoglobin was the first protein shown to be modified. Subsequent studies have demonstrated a similar effect of glucose on lens crystalline, collagen, erythrocyte membrane proteins, albumin and other serum proteins.
Measurement of these non-enzymatic glycosylated serum proteins (GSP) showed that the levels were approximately two to three times higher in diabetic than non-diabetic subjects. In diabetic patients the mean + SD of GSP was 0.693 + 0.257 (SD) nmol HMF per mg protein compared with 0.355 + 0.108 (SD) in controls
(P< 0.001). in diabetic patients levels appear to reflect glycemia in the preceding several days and correlate with other parameters of diabetes such as FBS. In this study non-enzymaticaly glycosylated serum protein measured by a colorimetric assay in 96 diabetic and 37 control subjects and relationship of GSP to FBS was shown.

A Case Report of Control of Vertigo in Meniere’s Syndrome after Endolymphatic Sac Surgery

Meniere’s syndrome is the most common cause of true vertigo that in 75-90% of the cases can be acceptably controlled by various methods of medical treatment. But in this case in spite of 13 years medical therapy, recurrent attacks of vertigo continue to occur and the patient completely become disable. During the last month, six severe vertigo attacks recorded by patients before surgery. So the patient was labeled as “intractable Meniere’s Disease” and accepted as good candidate for endolymphatic sac enhancement.
During 8 month after only 4 attacks of vertigo were recosded by the patient and recent audiogram show 20 dB raise in speech reception threshold and 12% improvement in speech discrimination score. Tinnitus is subjectively better and mild positional vertigo on rapid movements of head and mild sensation of imbalance in walking are only present complains of the patient. In this way, we can take patients back with meniere’s syndrome to near life by meticulous patient using this technique of surgery.

A Case Report of Precocious Pseudopuberty in A 4-Year-Old Girl

Precocious puberty may be classified as true precocious puberty or precocious pseudopuberty. The latter one in females may be in sexual (feminization) or hereto sexual (virilization). Ovarian tumors may be iso sexual call tumor, teratoma, etc, are among the etiologic factors of precocious psudopuberty. This paper presents a 4-year girl was admitted to the hospital with severe abdominal pain. Physical examination revealed generalized pertonitis and signs of precocious pseudo puberty including bilateral breast development, appearance of hair in face, trunk, external genitalia and lower extremity. Laparatomy confirmed the diagnosis of a right ovation tumor measuring 15x20cm and pathologist report stated granulosa cell tumor with intraparenchymal necrosis and hemorrhage. The age and clinical presentation of this patient, with regard to rare occurrence of G.C.T in pre-pubertal years (5%) make her as an unusual case.
