fa بررسی عوامل مؤثر در تأخیر مراجعه به بیمارستان پس از وقوع استروک حاد Factors Delaying Hospital Arrival Aftr Acute Stroke Onset سایر تخصص هاي باليني Other Clinical Specialties پژوهشي Original مقدمه: مراجعه به موقع پس از استروک و استفاده از درمانهای نوین همانند r-tPA وریدی و کنترل فشارخون باعث تسریع در بهبود علائم پس از سکته مغزی می‌شود. هدف از مطالعه حاضر تعیین عوامل مؤثر در تأخیر مراجعه به بیمارستان پس از وقوع استروک حاد در بیماران است. روش کار: در این مطالعه توصیفی تحلیلی 425 بیمار که پس از وقوع استروک به بیمارستان فرشچیان همدان در سالهای 1394-1395 مراجعه کرده بودند، وارد مطالعه شدند. اطلاعات دموگرافیک، مدت زمان مراجعه به مرکز درمانی، زمان وقوع استروک، ریسک فاکتورها، علایم بالینی استروک، نحوه ارجاع، فاصله تا مرکز درمانی و نوع استروک با استفاده از پرسشنامه و چک لیست جمعآوری شد. نتایج با استفاده از نرمافزار SPSS ویرایش 16 و توسط آزمونهای آماری ANOVA، X2 و کروسکال والیس مورد تجزیه و تحلیل قرار گرفت. یافته ها: میانگین سنی افراد شرکتکننده در مطالعه 12/70 سال بود.260 نفر (2/61 درصد) از افراد شرکتکننده مرد بودند. 6/85 درصد استروک از نوع ایسکمیک داشتند. میانگین زمانی مراجعه به اولین مرکز درمانی و بیمارستان فرشچیان به ترتیب 06/2 و 77/4 ساعت بود. 2/36 درصد افراد کمتر از 2 ساعت پس از وقوع سکته مغزی به بیمارستان فرشچیان مراجعه کردند. جنسیت مرد، سکته از نوع ایسکیمیک، تحصیلات پایین، فاصله بیشتر از مرکز درمانی، ارجاع از سایر مراکز درمانی، زندگی مجردی، نبود شاهد، سکته هنگام شب، سکته در منزل از عواملی بودند که باعث تأخیر دررسیدن به مرکز درمانی می‌شد. شایع‌ترین علامت بالینی در بیمارانی که کمتر از 2 ساعت مراجعه کرده بودند کنفوزیون و کاهش سطح هوشیاری با فراوانی 2/29 درصد بود.4/61 درصد از بیمارانی که کمتر از 2 ساعت مراجعه داشتند، فشار خون بالا شایع‌ترین عامل خطر در بین آنها بود. نتیجه‌گیری: بر اساس نتایج مطالعه حاضر زمان مراجعه جهت درمان بیماران پس از بروز سکته مغزی از میانگین بالایی برخوردار است عواملی چون فاصله زیاد از مرکز درمانی، نداشتن آگاهی کافی در مورد علائم استروک، رخداد استروک درشب، عدم مراجعه مستقیم به مرکز تخصصی، زندگی به تنهایی و نبود شاهد در زمان سکته از عوامل احتمالی تأخیر پس از بروز سکته به مراکز درمانی است. Introduction: Optimal time of referral after stroke and the use of new therapies, such as r-tPA and blood pressure control, could accelerate symptoms recovery after stroke. The aim of this study was to investigate factors contributing to the delay in referral to hospitals after the occurrence of acute stroke. Methods: This analytic-descriptive study included 425 patients, who had referred to the Hamadan Farshchian hospital during years 2015 and 2016. Demographic data, time of referring to medical center, the time of occurrence of stroke, risk factors, clinical symptoms of stroke, way of referring, distance from medical center and type of stroke by using a questionnaire and checklist were recorded. The SPSS (version 16) software was used for all calculations. Results: The mean age of patients in this study was 70.12 years old. Overall, 260 patients (61.2%) were male and 85.6% had ischemic stroke. Mean time of referral to first treatment center and Farshchian hospital was 2.06 and 4.77 hours, respectively. A total of 36.2% patients arrived to Farshchian hospital, within less than 2 hours. Factors that prolonged time to presentation were male gender, ischemic stroke, low education, being at a location far from the medical center, referral from other medical centers, being single, having no witness during stroke, and having a stroke at home, and during night. The most common symptoms in patients who were referred within less than two hours were confusion and loss of consciousness with frequency of 29.2%. The most common risk factor in 61.4% of patients, who were referred within less than 2 hours, was high blood pressure. Conclusion: This study showed mean time of referral for treatment after stroke was high.
