<strong>مقدمه</strong>: موکورمایکوز یک بیماری نادر و کشنده است که عمدتاً محدود به افراد دیابتی یا دارای نقص ایمنی می باشد. اشکال مختلف بیماری شامل: درگیری رینوسربرال، ریوی، پوستی، گوارشی، و عفونت منتشر می باشد. هدف از این گزارش معرفی یک مورد نادر از بیماری همراه با التهاب گوش میانی می باشد. </p><p align="justify" style="direction: rtl"><strong>معرفی بیمار</strong>: زنی 22 ساله با سابقه دیابت نوع 1 و نارسایی کلیه از کودکی که تحت درمان با انسولین و هفته ای دو نوبت همودیالیز بوده است ، بدلیل درد شدید و ناگهانی گوش چپ مراجعه نموده و آنتی بیوتیک خوراکی برای وی تجویز شد. روز بعد بدلیل تشدید علایم و فلج فاسیال سمت چپ بستری و تحت عمل میرنگوتومی، و آنتی بیوتیک وریدی قرار گرفت. در حین درمان دچار فلج اعصاب کرانیال 5 ،6 در سمت چپ و بدنبال آن تب و کاهش سطح هوشیاری، ادم نیمه چپ صورت و ترشح خونابه ای از چشم و بینی شد و علیرغم درمان با آنتی بیوتیک و آمفوتریسین بدنبال شوک سپتیک مقاوم به درمان درگذشت.</p><p align="justify" style="direction: rtl"> <strong>نتیجه نهایی</strong>: عفونت حاد گوش میانی در بیمار دیابتی ممکن است ناشی از موکورمایکوز باشد که با توجه به پیشرفت سریع نکروز و خطر مرگ لازم است هر چه سریعتر اقدامات تشخیصی و درمانی برای عفونت بعمل آید.

<strong>Introduction</strong>: Mucormycosis is a rare, fatal infection which mainly affects the patients with diabetes or immunodeficiency. Various clinical forms of the disease include rhinocerebral, pulmonary, cutaneous, gastrointestinal, and disseminated infection. The aim of this study was to report a rare case of mucormycosis involving middle ear.</p><p align="justify"><strong>Case Report</strong>: A 22-year-old female patient with a history of type 1 diabetes mellitus and end stage of renal disease who were under treatment with insulin and twice a week hemodialysis refered to hospital because of abrupt onset of severe left ear pain, and received oral antibiot-ics. Her symptoms worsened and left facial palsy developed the day after. Myringotomy was done and administration of parenteral antibiotics were begun. Subsequently, she developed left fifth and sixth cranial nerve palsies, followed by fever, unconsciousness, left hemi-facial edema, and serosanguinous discharge from eye and nose. In spite of parenteral antibacterial and antifungal therapy, she developed refractory septic shock and died. </p><p align="justify"><strong>Conclusion</strong>: Acute otitis media in diabetic patients may be due to mucormycosis. Considering the rapid progression of necrosis and fatality, prompt diagnosis and therapy for mucormycosis is mandatory.
