<div style="text-align: justify;"><span style="font-family:tahoma;"><strong><span style="position:relative;top:-1.0pt;letter-spacing:.1pt;"><span style="color:#274CC3;"><span style="font-size:10.0pt;">سابقه:</span></span></span></strong> <span style="font-size:10.0pt;">ایست قلبی در حین بیهوشی پدیدهای بالقوه کشنده</span> <span style="font-size:10.0pt;">و نادر است که امروزه علیرغم وجود داروها، تجهیزات و مانیتورینگهای پیشرفته هنوز بهعنوان یک مسأله مهم برای متخصصان بیهوشی مطرح میباشد.</span><strong><span style="letter-spacing:.1pt;"><span style="font-size:10.0pt;"></span></span></strong><br>
<strong><span style="position:relative;top:-1.0pt;"><span style="color:#274CC3;"><span style="font-size:10.0pt;">معرفی بیمار:</span></span></span></strong> <span style="font-size:10.0pt;">بیمار مردی 38 ساله، گچکار و معتاد به مواد مخدر بود که برای انجام جراحی لامینکتومی بستری</span> <span style="font-size:10.0pt;">شده بود. در شرح حال بیمار سابقه هیچگونه بیماری و مصرف دارو ثبت نگشته بود. اینداکشن بیهوشی با تجویز وریدی 2 میلیگرم میدازولام، 10 میکروگرم سوفنتانیل و 300 میلیگرم تیوپنتال سدیم همراه با 30 میلیگرم آتراکوریوم بهصورت آهسته انجام شد. پس از اینداکشن، بیمار بهطور ناگهانی دچار تنگی نفس، برادیکاردی شدید و ایست قلبی گردید. در ادامه، بلافاصله عملیات احیای قلبی- ریوی آغاز گشت و بیمار اینتوبه شد. سرانجام، بیمار ثبات همودینامیک یافت و بهطور کامل بیدار و هوشیار گردید. لوله تراشه بیمار در اتاق عمل خارج گشت و بیمار به </span><span dir="LTR"><span style="font-size:9.0pt;">CCU</span></span><span style="font-size:10.0pt;"> منتقل شد. شایان ذکر است که تمام آزمایشات بیمار پس از ایست قلبی نرمال بود؛ از این رو با حال عمومی خوب مرخص شد. با توجه به اعتیاد و تجویز داروهای آتراکوریم و تیوپنتال سدیم به نظر میرسد که محتملترین علت ایست قلبی در حین بیهوشی ناشی از واکنشهای آلرژیک (آنافیلاکسی) بوده باشد.</span><strong><span style="position:relative;top:-1.0pt;"><span style="color:#274CC3;"><span style="font-size:10.0pt;"></span></span></span></strong><br>
<strong><span style="position:relative;top:-1.0pt;"><span style="color:#274CC3;"><span style="font-size:10.0pt;">نتیجهگیری:</span></span></span></strong> <span style="font-size:10.0pt;">گزارش حاضر بر این نکته تأکید دارد که همیشه متخصصان بیهوشی باید از مانیتورینگ مداوم بیمار و دردسترسبودن داروها و تجهیزات احیای قلبی- ریوی اطمینان حاصل نموده و آمادگی لازم برای تشخیص و برخورد مناسب با ایست قلبی ناگهانی حین بیهوشی را داشته باشند<strong>.</strong></span><span style="font-size:10.0pt;"></span></span></div>
<div style="text-align: justify;"><strong>Background:</strong> Cardiac arrest during general anesthesia is a rare and potentially fatal condition. Despite the progression of monitoring equipments and anesthetic mediations, this phenomenon is still an important problem for the anesthesiologists.<br>
<strong>Case Presentation:</strong> Herein, we reported a 38-year-old opium addict plasterer male scheduled for laminectomy. <em>He had no previous medical history</em> and took no regular medication. Anesthesia was slowly induced with the intravenous administration of midazolam 2 mg, sufentanil 10 μg, <em>sodium thiopental</em> 300 mg, and atracurium 30 mg. The patient suddenly experienced respiratory distress, severe bradycardia, and cardiac arrest. Therefore, he was immediately subjected to cardiopulmonary resuscitation and intubation. Finally, t<em>he patient </em>became <em>hemodynamically stable</em> and <em>fully</em> conscious and <em>alert</em><em>.</em> He was extubated<em> in operating room and transferred</em> to the Coronary Care Unit. <em>All laboratory tests were normal</em> after cardiac arrest. Therefore, <em>he was discharged</em> in a <em>good general condition.</em> With regard to patient’s addiction and administration of atracorum and <em>sodium thiopental</em>, allergic reactions (anaphylaxis<strong>) </strong>seem to be the most likely cause of cardiac arrest during anesthesia.<br>
<strong>Conclusion:</strong> This report emphasizes that anesthetists must always make sure of continuous monitoring and availability of resuscitation equipment and drugs. Moreover, they should be prepared to appropriately diagnose and manage sudden cardiac arrest during anesthesia.<span dir="RTL"></span></div>
