fa بررسی میزان بیان سرمی miR-21 در بیماران مشکوک به عفونت مایکوباکتریوم توبرکلوزیس Evaluation of Serum miR-21 Expression Level in Patients Suspected of Mycobacterium tuberculosis بيماريهاي عفوني Infectious Diseases پژوهشي Original سابقه و هدف: مایکوباکتریوم توبرکلوزیس پاتوژنی درون‌سلولی است که موضع و هدف اصلی آن ریه‌ها، سیستم تنفسی فوقانی و غشای مخاطی آن‌هاست که به عفونت حاد یا مزمن دستگاه تنفسی منجر می‌شود. هدف مطالعه حاضر بیان miR-21 در افراد مشکوک به عفونت مایکوباکتریوم توبرکلوزیس بوده است. <pre dir="RTL" style="text-align:justify;direction:rtl;unicode-bidi:embed;"> </pre> <pre dir="RTL" style="text-align:justify;direction:rtl;unicode-bidi:embed;"> مواد و روش‌‌ها: این مطالعه از نوع مورد شاهدی بود و نتایج حاصل بین دو گروه بیمار و سالم مقایسه شد. نمونه‌های سرمی از 200 فرد مشکوک به عفونت مایکوباکتریوم توبرکلوزیس طی دوره سه ساله از بیمارستان‌های شهر کرمان جمع‌آوری شدند و پس از استخراج miR-21، میزان بیان آن با استفاده از روش Real time PCR بررسی شد.</pre> یافته‌ها: تفاوت معنی‌داری در بیان سرمی ژن miR-21 در افراد اسمیر منفی و PPD منفی دارای علامت وجود نداشت، اما در افراد اسمیر مثبت و PPD منفی، PPD مثبت و اسمیر منفی، اسمیر مثبت و PPD مثبت، افزایش معنی‌داری در بیان ژن miR-21 وجود داشت (05/0P<). نتیجه‌گیری: این نتایج به روش‌های تشخیصی دقیق‌تر نیاز دارد و روش‌های مولکولی را در تشخیص عفونت مایکوباکتریوم توبرکلوزیس تأیید می‌کند؛ چراکه تشخیص دیرهنگام عفونت می‌تواند فرصت درمان مؤثر را از بین ببرد و جان افراد را به خطر بیندازد. همچنین با توجه به ارتباط میان miR-21 و این عفونت، می‌توان از مهارکننده‌های miR-21 نیز برای درمان بیماری استفاده کرد. Background and Objective: Mycobacterium tuberculosis is an intracellular pathogen, and the main target of this bacterium is the lungs, upper respiratory system, and their mucous membranes leading to acute or chronic infection of the respiratory tract. The purpose of this study was to investigate the microRNA 21 (miR-21) expression levels in the patients suspected of Mycobacterium tuberculosis. Materials and Methods: The present study was a case-by-case study, and the results were compared between the two groups of patients and healthy individuals. The serum samples were collected from 200 patients suspected of Mycobacterium tuberculosis during a 3-year period in the hospitals of Kerman, Iran. The expression level of the extracted miR-21 was analyzed using real-time polymerase chain reaction. <pre> Results: There was no significant difference in the serum miR-21 gene expression in the symptomatic patients with negative smear and negative purified protein derivative (PPD) skin test. However, the patients with positive smear and negative PPD, positive PPD and negative smear, and positive smear and positive PPD showed a significant increase in the miR-21 gene expression (P<0.05).</pre> Conclusion: The obtained results confirmed the need for more accurate diagnostic methods, especially molecular techniques, for the detection of M. tuberculosis infection as late detection of infection can eliminate the possibility of effective treatment and endanger the lives of individuals.
