fa بررسی سطح بیان پروتئین‌ PDL-1 در نمونه‌های بافتی بیماران مبتلا به سرطان معده Evaluation of PDL-1 Protein Expression in Tumor Tissues of Patients with Gasteric Cancer پزشكي ملکولی و بیوتکنولوژی Molecular Medicine & Biotechnology پژوهشي Original <div style="text-align: justify;">سابقه و هدف: امروزه ثابت شده است سیستم‌ایمنی نقش مهمی در ایجاد یا درمان برخی سرطان‌ها ایفا می‌کند. یکی از اجزای سیستم‌ایمنی، پروتئین‌های PDL-1 هستند که از سلول‌های مختلف بدن ازجمله سلول‌های B و سلول‌های T بیان می‌شوند. هدف از مطالعه حاضر تعیین فراوانی بیان پروتئین‌ PDL1 و ارتباط آن با مشخصات هیستوپاتولوژی تومور در مبتلایان به سرطان معده بود. مواد و روش‌‌ها: این گزارش شامل موارد بیماری (Case Series) 100 بیمار مراجعه‌کننده بود. بیماران بین سال‌های 1399-1395 به درمانگاه آنکولوژی <a>واقع در کلینیک امام خمینی</a><a class="msocomanchor" href="#_msocom_1" id="_anchor_1" language="JavaScript" name="_msoanchor_1" on<illegal tag>mouseout="msoCommentHide('_com_1')" on<illegal tag>mouseover="msoCommentShow('_anchor_1','_com_1')">[A1]</a>  شهر همدان مراجعه کرده بودند و سرطان معده با پاتولوژی بیوپسی به روش آندوسکوپی برایشان تشخیص داده شده بود. موارد ازنظر میزان بیان پروتئین PDL-1 با روش IHC بررسی شدند و ارتباط آنها را با سن، جنس، متاستاز، مرحله (Stage) و درجه (Grade)  تومور مطالعه کردیم. یافته‌ها: میانگین سن بیماران 10/83±65/53 سال، ازنظر جنسیت 72درصد مرد و 28درصد زن بودند. ازنظر نتیجه بیان پروتئین PDL-1 به‌ترتیب 2 نفر منفی، 25 نفر مثبت ضعیف و 73 نفر مثبت قوی بودند. از 100 بیمار 69 نفر دارای متاستاز، 69 نفر دارای درجه بالا و 68 نفر در مرحله 4 بیماری بودند. یک بیمار در  مرحله یک بیماری بود که بیان مثبت ضعیف <st1:stockticker w:st="on">PDL</st1:stockticker>1  داشت. 21 بیمار در مرحله دو بیماری بودند که ازنظر بیان <st1:stockticker w:st="on">PDL</st1:stockticker>1 دو مورد منفی، 14 مورد مثبت ضعیف، 5 مورد مثبت قوی بودند.10 بیمار در مرحله سه بودند که همگی آنها بیان مثبت ضعیف <st1:stockticker w:st="on">PDL</st1:stockticker>1 و از میان 68 بیمار مرحله چهار، همگی بیان مثبت قوی <st1:stockticker w:st="on">PDL</st1:stockticker>1  داشتند. میان بیان PDL-1 با متاستاز، درجه و مرحله بالاتر تومور ارتباط معنی‌دار مشاهده شد (0/05 P<). نتیجه‌گیری: بیشترِ بیماران مبتلا به سرطان معده، دارای بیان مثبت قوی پروتئین PDL-1 بودند. بیان 1PDL- ارتباط معنی‌داری با شدت سرطان معده ازجمله درجه و مرحله بالاتر و متاستاز داشت؛ بنابراین، از بیومارکر PDL-1 می‌توان به‌عنوان عامل تعیین‌کننده شدت و پروگنوز سرطان معده استفاده کرد. همچنین، دارو‌های با عملکرد آنتی PDL-1  در درمان سرطان معده می‌توانند مؤثر باشند.   <div> <hr align="left" class="msocomoff" size="1" width="33%" > <div> <div class="msocomtxt" id="_com_1" language="JavaScript" on<illegal tag>mouseout="msoCommentHide('_com_1')" on<illegal tag>mouseover="msoCommentShow('_anchor_1','_com_1')"> <a class="msocomoff" href="#_msoanchor_1">[A1]</a>This phrase and the following highlighted sentences were not translated; please consider translating them if you will.</div> </div> </div> </div> <div> <hr align="left" class="msocomoff" size="1" width="33%" > <div class="msocomtxt" id="_com_1" language="JavaScript" on<illegal tag>mouseout="msoCommentHide('_com_1')" on<illegal tag>mouseover="msoCommentShow('_anchor_1','_com_1')" style="text-align: justify;"><a name="_msocom_1"></a> <a class="msocomoff" href="#_msoanchor_1">[A1]</a>This phrase and the following highlighted sentences were not translated; please consider translating them if you will.</div> </div> <div style="text-align: justify;"></div> <div style="text-align: justify;">Background and Objective: Nowadays, it has been proven that the immune system plays a major role in the occurrence or treatment of some cancers. The PDL-1 proteins are among the components of the immune system, which are expressed in different body cells, including B cells and T cells. In this regard, the present study was conducted to determine the frequency of PDL-1 protein expression and its relationship with tumor histopathology in patients with gastric cancer. Materials and Methods: In this case series study, 100 patients referred to the Oncology Clinic, located in Imam Khomeini Clinic, Hamadan, Iran, between 2016-2021, with the diagnosis of gastric cancer (confirmed by endoscopic biopsy pathology) were selected. Moreover, their PDL-1 protein expression (with IHC) and its relationship with age, gender, metastasis, stage, and grade of the tumor were evaluated. Results: The average age of patients was 65.53 ± 10.83 years, and 72% were male. The result of PDL-1 protein expression showed that 2 patients were negative, 25 patients were weakly positive and 73 patients were strongly positive. In addition, 69 Out of 100 patients had metastases, 69 cases had high grade and 68 were in stage 4 of the disease. A significant relationship was observed between PDL-1 expression with metastasis, higher grade, and higher stage of the tumor (P<0.05). Conclusion: Most patients with gastric cancer had a strongly positive expression of PDL-1 protein.
