fa مقایسه دو روش بخیه‌ی زیر جلدی نیمه پلاستیکی ساده و مداوم در بهبود زخم جراحی آپاندکتومی Comparison of Two Methods of Simple and Continuous Plastic Subcutaneous Suture in Appendectomy Wound Healing جراحي عمومي General Surgery پژوهشي Original سابقه و هدف: آپاندکتومی یکی از رایج‌ترین روش‌های جراحی با عوارض بالینی مختلف است که بر بهبود زخم تأثیر می‌گذارد. نوع بسته‌شدن پوست (به جهت پیشگیری از عفونت و اسکار پوستی) یک مرحله‌ی حیاتی از جراحی به شمار می‌رود. این مطالعه به‌منظور مقایسه دو روش بخیه‌ زیرجلدی نیمه پلاستیکی سادهSimple Semi-Plastic (SSP)  و مداوم Continuous Sub-Cuticular (CSC)  در بهبود زخم جراحی آپاندکتومی انجام شده است. مواد و روش‌‌ها: این مطالعه کارآزمایی بالینی سه‌سوکور بر روی 73 بیمار آپاندکتومی که به‌طور تصادفی در دو گروه SSP (n=38) و CSC (n=35)  قرار گرفتند، انجام شد. داده‌های دموگرافیک، ویژگی‌های مرتبط با زخم (عرض اسکار، رنگ، بو، باز بودن، بافت نکروزه، گرمی زخم، وضعیت زخم ازنظر بافت‌های اپیتلیالیزه، مقدار و رنگ ترشحات زخم و رنگ بافت‌های اطراف زخم) به‌طور کامل با استفاده از چک‌لیست از پیش طراحی‌شده سنجیده شد. همچنین از مقیاس (VAS) نیز برای ارزیابی سطح درد استفاده گردید. تمامی داده‌ها در روزهای 2، 10 و 30 پس از جراحی مورد ارزیابی قرار گرفت. داده‌ها با استفاده از نرم‌افزار SPSS (نسخه 23) تجزیه‌وتحلیل شدند و سطح معنی‌داری P<0.05 در نظر گرفته شد. یافته‌ها: تفاوت معنی‌داری ازنظر مشخصات دموگرافیک، عوارض مرتبط با زخم، عوامل مرتبط با بهبود و تغییرات بافت‌های اطراف زخم بین گروه‌های SSP و CSC مشاهده نشد (P>0.05). عرض اسکار در روز دوم و دهم پس از بخیه زدن در گروه SSP به‌طور معنی‌داری بیشتر از افراد CSC بود (P<0.05). نتیجه‌گیری: پس از آپاندکتومی در روش بخیه‌ی CSC اسکارهای پوستی نازک‌تر به همراه نتایج زیبایی بهتر نسبت به بخیه‌ی SSP حاصل شد.   Background and Objective: Appendectomy is one of the most common surgical procedures with various clinical complications affecting wound healing. The type of skin closure is a critical surgical stage to prevent infection and dermal scarring. Therefore, administration of proper skin suturing can lead to complete wound healing and reduction of appendectomy-associated complications. In this regard, the present study was designed to compare the quality of the wound healing process and related complications in Simple Semi-Plastic (SSP) and Continuous Sub-Cuticular (CSC) dermal suturing procedures. Materials and Methods: This triple-blind clinical trial study was conducted on 73 appendectomy patients randomly categorized as the cases with SSP (n=38) and CSC (n=35) suturing procedures. Demographic data, wound-associated characteristics (e.g., width of scar, color, odor, patency, necrotic tissue, wound warmth, granulation and epithelialization tissues, volume and color of wound secretion, and color of wound-surrounded tissues) were totally assessed using a pre-designed checklist. Visual Analogue Scale (VAS) index was also used for assessing level of pain. All the collected data were applied on 2nd, 10th, and 30th days post-surgery. The obtained data was analyzed in SPSS software (version 23). Results: According to the results of the present research, no significant differences were detected in the terms of demographic characteristics, wound-associated complications, healing-related factors, and changes of wound-surrounding tissues between the SSP and CSC groups (P>0.05). The width of scar was significantly higher on the 2nd and the 10th days after suturing in SSP group than CSC group (P<0.05).
