مقایسه سطح سیتوکاین های IL-5، IL-10، IL-12 و IL-18 مترشحه از سلول های PBMC در بیماران مبتلا به لیشمانیوز پوستی بهبود یابنده و غیر بهبود یابنده

<strong>مقدمه و هدف</strong>: لیشمانیوز یک مشکل مهم بهداشتی در سراسر جهان از جمله کشور ما ایران می باشد. ضایعات پوستی عموماً خود بهبود یابنده اند اما اشکال غیر بهبود یابنده لیشمانیوز پوستی نیز اخیراً افزایش یافته است. القای پاسخ سلولهای نوعT-helper type1(Th1) به مقاومت در برابر بیماری کمک می کند در حالیکه پاسخ های helper type2 (Th2) T- باعث حساسیت در برابر بیماری می شود. با ارزیابی سیتوکاینهای (IL-5)، (IL-10)، (IL-12) و (IL-18) مترشحه از سلولهای مونونوکلئر خون محیطی (PBMC) در بیماران لیشمانیوز پوستی بهبود یابنده و غیر بهبود یابنده، نقش این سیتوکاینها را در بهبود بیماران بررسی می کنیم.
</p><p align="justify" style="direction: rtl"><strong>روش کار</strong>: سطح سیتوکاینهای مترشحه از سلولهای PBMC در 60 نفر از بیماران بهبودیابنده و غیربهبودیابنده و گروه کنترل درمانگاه شماره یک آب و برق و بیمارستان قائم مشهد در سال 1385 پس از تحریک با آنتیژن لیشمانیا ماژور و میتوژن در محیط in vitro توسط روش الیزا و استفاده از کیت های صنعتی ارزیابی شد.</p><p align="justify" style="direction: rtl">
<strong>نتایج</strong>: سلولهای PBMC افراد بهبود یافته، IL-12 را با غلظت 38.00±236.55 و بیش از بیماران غیربهبودیابنده ترشح کردند (P<0.05) درحالیکه در بیماران غیربهبودیابنده IL-5 (pg/ml 52.14±65.21) و IL-10 (pg/ml 30.19±18.73) بیش از بیماران بهبود یافته ترشح شدند (P<0.005). همچنین دریافتیم IL-18 در بیماران غیربهبودیابنده با غلظت pg/ml 433.02±225.30 به طور معنی داری بیش از افراد بهبود یافته ترشح میشود (P=0.003).
</p><p align="justify" style="direction: rtl"><strong>نتیجه نهایی</strong>: با توجه به نتایج بدستآمده میتوان نتیجه گرفت که IL-12 در افراد بهبود یافته نسبت به افراد غیر بهبود یافته بیشتر ترشح می شود ولیIL-18 که باعث افزایش ترشح
IL-12 و فعالیت سلولهای Th1 میشود در مواقعی که ترشح IL-12 کاهش مییابد، در افراد غیر بهبودیافته بیشتر ترشح می شود و در آنها باعث القاء پاسخهای

<strong>Introduction & Objective</strong>: Leishmaniasis is a major worldwide health problem throughout the world and in Iran as well. Skin lesions are mostly healing but nonhealing cases have also increased recently. T-helper type1 (Th1) responses induction helps the patients to resist against the disease while T-helper type2 (Th2) responses cause susceptibility to disease. With the assessment of IL-5, IL-10, IL-12, and IL-18 cytokines secreted from peripheral blood mononuclear cells (PBMC) in leishmaniasis healing and nonhealing patients, their role in curing leishmaniasis was surveyed.
</p><p align="justify"><strong>Materials & Methods</strong>: Cytokines secreted from PBMCs of 60 healing and nonhealing patients and also control group referred to Ab-o-Bargh number1 clinic and Mashhad Qaem hospital during 2007 were evaluated Elisa commercial kits after stimulation by leishmania antigen and mitogen in vitro.
</p><p align="justify"><strong>Results</strong>: Healing patients PBMCs secreted IL-12 (236.55±38.00 pg/ml) more than non healing patients (p<0.05) while in nonhealing patients IL-5 (52.14±65.21 pg/ml) and IL-10 (30.19±18.73 pg/ml) secreted more than healing patients (p<0.005). Also IL-18 secreted significantly higher in nonhealing patients compared with healing patients (p=0.003).
</p><p align="justify"><strong>Conclusion</strong>: According to the results It can be concluded that IL-12 is secreted more in healing patients than nonhealing patients but IL-18, which causes the increase of IL-12 secretion and the activation of Th1 cells during IL-12 secretion decrease, secrets more in nonhealing patients, induces Th2 responses and results in disease progression
