fa بررسی عوارض استخوانی مفصلی بیماران مبتلا به تب مالت بستری در بیمارستان: یک مطالعه ده ساله Bone and Joint Complications in Hospitalized Brucellosis Patients: A 10-Year Study in Hamadan, Iran بيماريهاي عفوني Infectious Diseases پژوهشي Original سابقه و هدف: بروسلوز یک بیماری مشترک بین انسان و دام است که در خاورمیانه از جمله ایران شایع است. مجموعه علائم اسکلتی عضلانی، به ویژه در ستون فقرات، یکی از علائم شایع بروسلوز است که بیشتر به دلیل علائم مختلف و غیر اختصاصی منجر به تاخیر در تشخیص بیماری می‌شود. این مطالعه با هدف تعیین عوارض اسکلتی و مفصلی در بیماران بستری در بیمارستان با تشخیص تب مالت انجام شد. مواد و روش‌‌ها: این مطالعه گذشته‌نگر، سوابق پزشکی ۷۸۲ بیمار مبتلا به بروسلوز بستری در بیمارستان سینا همدان را بین سال‌های ۱۳۹۰ تا ۱۳۹۹بررسی کرد. یافته‌ها: تعداد زیادی (87/6 ٪) از بیماران سابقه مصرف لبنیات غیرپاستوریزه و بیش از نیمی (57/2 %) در سال گذشته با دام مواجهه داشته‌اند. شایع‌ترین نشانه‌های بالینی شامل ضعف، خستگی، درد مفاصل و تب بوده است، در حالی که درد کمر و زانو درد علائم اسکلتی عضلانی غالب بوده‌اند. آرتریت (24/4 %)، اسپوندیلیت (15/4 %) و ساکروایلیت (13/4 %) عوارض اصلی اسکلتی عضلانی شناسایی شده بودند. نتیجه‌گیری: مجموعه‌علائم بالینی بروسلوز علائم شایعی مانند تب، ضعف، خستگی و درد مفاصل را نشان می‌دهد که در بیماران مرد شیوع بیشتری دارد. عوارض اسکلتی عضلانی، از علائم بارز این بیماری هستند. بر اساس نتایج اسکن استخوان و MRI حدود یک پنجم بیماران بستری مبتلا به بروسلوز دچار عوارض استخوانی مفصلی از جمله آرتریت، اسپوندیلیت و ساکروایلیت بوده‌اند.   Background and Objective: Brucellosis is a zoonotic disease common in the Middle East, including Iran. Musculoskeletal manifestations, particularly in the spine, are frequent symptoms of brucellosis, often leading to delays in diagnosis due to its varied and non-specific symptoms. The present study aimed to investigate the prevalence, clinical presentation, and diagnostic challenges of musculoskeletal brucellosis, focusing on spinal involvement. Materials and Methods: This retrospective study reviewed the medical records of 782 patients with brucellosis admitted to Sina Hospital in Hamadan, Iran, from 2011 to 2020. Results: The results indicated that a significant portion (87.6%) of the patients had a history of consuming unpasteurized dairy products, and more than half (57.2%) had been exposed to livestock in the past year. The most common clinical symptoms were weakness and fatigue (66.7%), joint pain (62.7%), and fever (73.1%), while back pain (65.7%) and knee pain (48.3%) were the dominant musculoskeletal symptoms. In addition, arthritis (24.4%), spondylitis (15.4%), and sacroiliitis (13.4%) were the primary identified musculoskeletal complications. Conclusion: The clinical manifestations of brucellosis include common symptoms, such as fever, weakness, fatigue, and joint pain, which were more prevalent in male patients. In addition, musculoskeletal complications are prominent features of the disease.
