fa عملکرد قلبی با تأکید بر شاخص GLS در بیماران پارکینسون تحت تحریک عمقی مغزی: مقایسه‌ بین بیماران کاندید و دریافت‌کننده DBS Cardiac Function with Emphasis on the GLS Index in Parkinson’s Disease Patients Undergoing Deep Brain Stimulation: A Comparative Study between DBS Candidates and Recipients قلب و عروق Cardiovascular Diseases پژوهشي Original <div style="text-align: justify;">سابقه و هدف: بیماری پارکینسون پس از آلزایمر، دومین بیماری شایع نورودژنراتیو است. تحریک عمقی مغزی (Deep Brain Stimulation; DBS) در این بیماران ممکن است در عملکرد سیستم قلبی‌عروقی تأثیر داشته باشد. هدف این مطالعه مقایسه شاخص‌های عملکرد قلبی ازجملهGLS  (Global Longitudinal Strain) در بیماران پارکینسون کاندید و دریافت‌کننده تحریک عمقی مغزی بود. مواد و روش‌‌ها: در این مطالعه، یک کوهورت مشاهده‌ای آینده‌نگر روی 37 بیمار بالای ۱۸ سال مبتلابه پارکینسون کاندید جراحی DBS انجام شد. بیماران دریافت‌کنندهDBS  با کاندیدهایی که هنوز جراحی را انجام نداده بودند، مقایسه شدند. شاخص‌های قلبی مانند GLS، LVEDV، LVEF و دیگر پارامترهای اکوکاردیوگرافی بررسی شدند. از آزمون‌های testt-، کای اسکوئر و دقیق فیشر برای تحلیل داده‌ها استفاده شد. یافته‌ها: میانگین سن شرکت‌کنندگان 16.9±57.7 سال بود (دامنه 29 تا 89 سال). از 37 بیمار، 14 نفر زن (37.8 درصد) و 23 نفر مرد (62.2 درصد) بودند. تفاوت معنی‌داری در میانگین GLS، LVEDV، LVESV، LVEF، SPAP، E/e، E و A بین دو گروه مشاهده نشد)0.05(P>. بااین‌حال، میانگین e' در گروه دریافت‌کننده DBS به‌طور معنی‌داری بالاتر بود (0.66±9.62 در مقایسه با 0.8±9.08؛ 0.046=P).   نتیجه‌گیری: نتایج نشان داد که شاخص‌های اصلی عملکرد قلبی ازجمله GLS در بیماران با و بدون تحریک مغزی تفاوت معنی‌داری ندارند، اما بهبود معنادار پارامتر e' در گروه DBS بیانگر بهبود عملکرد دیاستولیک قلب در این بیماران است.</div> <div style="text-align: justify;">Background and Objective: Parkinson’s disease is the second most common neurodegenerative disorder after Alzheimer’s disease. Deep brain stimulation (DBS) in these patients may affect cardiovascular system function. This study aimed to compare cardiac function indices, including Global Longitudinal Strain (GLS), between Parkinson’s disease patients who are candidates for DBS and those who are receiving DBS. Materials and Methods: This prospective observational cohort study involved 37 Parkinson’s patients aged 18 years or older who were candidates for DBS surgery. Patients who had received DBS were compared with those who had not yet undergone the procedure. Cardiac indices, such as GLS, LVEDV, LVEF, and other echocardiographic parameters, were assessed. Data were analyzed using Student's t-test, chi-square, and Fisher's exact tests. Results: The mean age of participants (n=37) was 57.7 ± 16.9 years (age range: 29 to 89), and the majority of the patients (n=23, 62.2%) were male. No significant differences were found between the two groups in terms of mean GLS, LVEDV, LVESV, LVEF, SPAP, E/e', E, and A (P > 0.05). However, the mean e' was significantly higher in the DBS group (9.62 ± 0.66 vs. 9.08 ± 0.8, P = 0.046). Conclusion: The leading cardiac function indices, including GLS, did not differ significantly between patients with and without DBS.
