fa تعیین میزان نیاز به خون در جراحی بای پس عروق کرونر بر حسب شرایط بیمار و فاکتورهای قبل از عمل Predicting Blood Transfusion Factors in Coronary Artery Bypass Surgery سایر تخصص هاي باليني Other Clinical Specialties پژوهشي Original <p dir="rtl" style="color: rgb(45, 135, 179) font-family: tahoma font-size: 12px font-style: normal font-variant: normal font-weight: normal letter-spacing: normal line-height: 20px orphans: auto text-indent: 0px text-transform: none white-space: normal widows: 1 word-spacing: 0px -webkit-text-stroke-width: 0px text-align: justify background-color: rgb(247, 246, 243)">مقدمه و هدف : توانائی پیشگوئی میزان مصرف خون حین عمل جراحی عروق کرونر باعث بهبود توانائی مراکز انتقال خون در تأمین خون سالم و کافی می شود . به همین منظور این مطالعه با هدف تعیین فاکتورهای پیشگویی در میزان نیاز به خون در جراحی بای پس عروق کرونر انجام گرفت. <p dir="rtl" style="color: rgb(45, 135, 179) font-family: tahoma font-size: 12px font-style: normal font-variant: normal font-weight: normal letter-spacing: normal line-height: 20px orphans: auto text-indent: 0px text-transform: none white-space: normal widows: 1 word-spacing: 0px -webkit-text-stroke-width: 0px text-align: justify background-color: rgb(247, 246, 243)">روش کار : یک مطالعه مقطعی گذشته نگر بر روی کلیه بیمارانی که در سال 1383 در بخش جراحی قلب بیمارستان نمازی شیراز تحت عمل جراحی بای پاس عروق کرونر قرار گرفته بودند انجام گرفت ، حجم نمونه 480 نفر و نمونه گیری به صورت تصادفی سیستماتیک انجام شد. فرم اطلاعات در مورد عوامل مؤثر در میزان نیاز به خون توسط پژوهشگر پر شد. از تست آماری Independent t-test وMultivariate logistic regression و همبستگی پوآسن جهت آنالیز داده ها استفاده شد و P کمتر از 0.05 معنی دار تلقی گردید . <p dir="rtl" style="color: rgb(45, 135, 179) font-family: tahoma font-size: 12px font-style: normal font-variant: normal font-weight: normal letter-spacing: normal line-height: 20px orphans: auto text-indent: 0px text-transform: none white-space: normal widows: 1 word-spacing: 0px -webkit-text-stroke-width: 0px text-align: justify background-color: rgb(247, 246, 243)">نتایج : میانگین سنی بیماران 59.15±10.69 سال بود که 69.2% مرد بودند. میانگین هماتوکریت قبل از عمل 39.86±6.38 و بعد از عمل 32.68±5.84 بود و متوسط مدت عمل 2 ساعت و 48 دقیقه بود . در این مطالعه میزان مصرف گلبول قرمز فشرده و FFP جهت انجام عمل جراحی عروق کرونر به ترتیب 5.76±1.5 کیسه و 2.82±7.72 کیسه بود. در این مطالعه میزان مصرف خون و FFP ارتباطی به سن ، وزن ، هماتوکریت قبل از عمل ، جنس و مدت جراحی نداشت (P>0.05). میزان مصرف خون فشرده و FFP در عمل های جراحی اورژانس ، ابتلا به دیابت و وجود بیماری های عروق محیطی بیشتر بود (P<0.05). <p style="color: rgb(45, 135, 179) font-family: tahoma font-size: 12px font-style: normal font-variant: normal font-weight: normal letter-spacing: normal line-height: 20px orphans: auto text-align: -webkit-right text-indent: 0px text-transform: none white-space: normal widows: 1 word-spacing: 0px -webkit-text-stroke-width: 0px background-color: rgb(247, 246, 243)">نتیجه نهائی : به طور کلی چنین به نظر می رسد میزان مصرف خون در جراحی عروق کرونر در کشور ما نسبت به سایر کشورها بیشتر باشد. با توجه به خطرات عفونی و ایمونولوژیک تزریق خون ، جراحان قلب می بایست در زمینه تصمیم گیری در میزان خون لازم دقت بیشتری مبذول دارند. <p style="color: rgb(45, 135, 179) font-family: "Times New Roman" font-size: 16px font-style: normal font-variant: normal font-weight: normal letter-spacing: normal line-height: 22.6667px text-indent: 0px text-transform: none white-space: normal widows: 1 word-spacing: 0px direction: ltr unicode-bidi: embed background-color: rgb(247, 246, 243) text-align: justify">Introduction & Objective: The ability to predict the use of blood components during bypass surgery will improve the blood banks ability to provide efficient service. <p style="color: rgb(45, 135, 179) font-family: "Times New Roman" font-size: 16px font-style: normal font-variant: normal font-weight: normal letter-spacing: normal line-height: 22.6667px text-indent: 0px text-transform: none white-space: normal widows: 1 word-spacing: 0px direction: ltr unicode-bidi: embed background-color: rgb(247, 246, 243) text-align: justify">Materials and Methods: This is a retrospective cross-sectional study that was carried out on patients that underwent CABG in Shiraz Namazi hospital in 2004. The sample size was 480 and we did systematic random sampling, and a questionnaire contained factors that effect on blood need was filling out. Independent t-test, multivariate logistic regression and Poason correlation were used for data analysis. <p style="color: rgb(45, 135, 179) font-family: "Times New Roman" font-size: 16px font-style: normal font-variant: normal font-weight: normal letter-spacing: normal line-height: 22.6667px text-indent: 0px text-transform: none white-space: normal widows: 1 word-spacing: 0px direction: ltr unicode-bidi: embed background-color: rgb(247, 246, 243) text-align: justify">Results: The average age of patients was 59.15±10.6 years, 69.2% were male and the mean Hct before surgery was 39.86±6.38% and after operation was 32.68±5.84% and the duration of surgery was 2 hours and 48 minutes. The average of pack cell was 5.76±1.52 unit and average of FFP was 2.82±7.72 unit. There was not significant correlation between ages, weighs, Hct before and after, sex and pack cell use (P> 0.05).
