fa بررسی اثرات درمانی سورفاکتانت در کاهش مرگ و میر نوزادان Evaluation of Surfactant Effects on Newborns سایر تخصص هاي باليني Other Clinical Specialties پژوهشي Original <p dir="rtl" style="color: rgb(45, 135, 179) font-family: tahoma font-size: 12px font-style: normal font-variant: normal font-weight: normal letter-spacing: normal line-height: 20px text-indent: 0px text-transform: none white-space: normal widows: 1 word-spacing: 0px background-color: rgb(247, 246, 243) text-align: justify">مقدمه و هدف: یکی از درمانهای استاندارد نوزادان با سندرم دیسترس تنفسی (Respiratory Distress Syndrome RDS) تجویز سورفاکتانت است که باعث کاهش مورتالیته و پنوموتوراکس و نیاز به تهویه مکانیکی می شود. این مطالعه با هدف تعیین اثرات درمانی سورفاکتانت در کاهش مرگ و میر نوزادان انجام گرفت. <p dir="rtl" style="color: rgb(45, 135, 179) font-family: tahoma font-size: 12px font-style: normal font-variant: normal font-weight: normal letter-spacing: normal line-height: 20px text-indent: 0px text-transform: none white-space: normal widows: 1 word-spacing: 0px background-color: rgb(247, 246, 243) text-align: justify">روش کار: این مطالعه توصیفی تحلیلی در NICU بیمارستان حضرت علی اصغر تهران در فواصل سالهای 1373 تا 1374 و 1381 تا 1382 جهت مقایسه دو گروه نوزادان مبتلا به RDS شدید با تهویه مکانیکی در 24 ساعت اول که در یک گروه با سورفاکتانت درمان شده و در گروه دیگر با سورفاکتانت درمان نشده بودند و حمایت تهویه مکانیکی دریافت کرده بودند، انجام گردید. در این مطالعه 36 نوزاد درمان شده با سورفاکتانت و 52 نوزاد که تنها تهویه مکانیکی شده بودند با هم مقایسه شدند. داده ها از پرونده ها جمع آوری و بانرم افزار SPSS و با استفاده از آزمون آماری آنالیز گردید. <p dir="rtl" style="color: rgb(45, 135, 179) font-family: tahoma font-size: 12px font-style: normal font-variant: normal font-weight: normal letter-spacing: normal line-height: 20px text-indent: 0px text-transform: none white-space: normal widows: 1 word-spacing: 0px background-color: rgb(247, 246, 243) text-align: justify"> نتایج: مورتالیته در بیماران درمان شده با سورفاکتانت بطور معناداری پایینتر بود. اختلاف معناداری در بروزIVH، بروز پنوموتوراکس، بروز سپسیس، مدت بستری و نیاز به ونتیلاسیون بین دو گروه وجود نداشت. <p style="color: rgb(45, 135, 179) font-family: tahoma font-size: 12px font-style: normal font-variant: normal font-weight: normal letter-spacing: normal line-height: 20px text-indent: 0px text-transform: none white-space: normal widows: 1 word-spacing: 0px background-color: rgb(247, 246, 243) text-align: justify">نتیجه نهایی: نتایج این مطالعه لزوم تجویز سورفاکتانت در موارد RDSشدید را در کشور ما و توسط یک تیم پزشکی با تجربه نشان می دهد. <span id="ctl00_ContentPlaceHolder1_DataList2_ctl00_abstractLabel0" style="color: rgb(45, 135, 179) font-style: normal font-variant: normal font-weight: normal letter-spacing: normal orphans: auto text-align: -webkit-left text-indent: 0px text-transform: none white-space: normal widows: 1 word-spacing: 0px -webkit-text-stroke-width: 0px font-family: 'Times New Roman' font-size: 12pt line-height: 17pt background-color: rgb(247, 246, 243)">Introduction & Objective: One of the standard therapies in neonates with severe respiratory distress syndrome (RDS) is surfactant administration in early course of therapy that cause reduction in mortality, pneumothorax and need to mechanical ventilation. In this study that was carried out in Aliasghar Hospital NICU in 1994-1995 & 2001-2002, the goal was to compare two groups of neonates with severe RDS that had been ventilated in the first 24 hours but one group had received surfactant and the other group (7 years ago) was deprived of this substance. <span id="ctl00_ContentPlaceHolder1_DataList2_ctl00_abstractLabel0" style="color: rgb(45, 135, 179) font-style: normal font-variant: normal font-weight: normal letter-spacing: normal orphans: auto text-align: -webkit-left text-indent: 0px text-transform: none white-space: normal widows: 1 word-spacing: 0px -webkit-text-stroke-width: 0px font-family: 'Times New Roman' font-size: 12pt line-height: 17pt background-color: rgb(247, 246, 243)">Materials & Methods: In our study, 36 neonates that received surfactant and 52 neonates with only mechanical ventilation therapy were compared. Data collected and analyzed using SPSS. <span id="ctl00_ContentPlaceHolder1_DataList2_ctl00_abstractLabel0" style="color: rgb(45, 135, 179) font-style: normal font-variant: normal font-weight: normal letter-spacing: normal orphans: auto text-align: -webkit-left text-indent: 0px text-transform: none white-space: normal widows: 1 word-spacing: 0px -webkit-text-stroke-width: 0px font-family: 'Times New Roman' font-size: 12pt line-height: 17pt background-color: rgb(247, 246, 243)">Results: We found that mortality in patients with surfactant administration was significantly lower compared to the second group who did not receive surfactant.
