<p dir="rtl" style="color: rgb(45, 135, 179) font-family: tahoma font-size: 12px font-style: normal font-variant: normal font-weight: normal letter-spacing: normal line-height: 20px text-indent: 0px text-transform: none white-space: normal widows: 1 word-spacing: 0px background-color: rgb(247, 246, 243) text-align: justify"><strong><span style="font-size: 9pt">مقدمه و هدف:</span></strong><font size="2"> اختلالات الکترولیتی در مرحله حاد خونریزی ساب آراکنوئید (</font><span dir="ltr" style="font-size: 9pt">subarachnoid hemorrhage</span><font size="2">) یا </font><span dir="ltr" style="font-size: 9pt">SAH</span><font size="2"> غالباً مشاهده میشود و هیپوناترمی و هیپرناترمی جزء شایعترین این اختلالات الکترولیتی در مرحله حاد بیماری میباشند که این اختلالات میتواند باعث بدتر شدن ادم مغزی و اثرفشاری شود. در این مطالعه رابطه اختلالات سطح سرمی سدیم با پیش آگهی بیماری بررسی شده است.</font></p>
<p dir="rtl" style="color: rgb(45, 135, 179) font-family: tahoma font-size: 12px font-style: normal font-variant: normal font-weight: normal letter-spacing: normal line-height: 20px text-indent: 0px text-transform: none white-space: normal widows: 1 word-spacing: 0px background-color: rgb(247, 246, 243) text-align: justify"><strong><span style="font-size: 9pt">روش کار :</span></strong><font size="2"> این مطالعه توصیفی تحلیلی به صورت آیندهنگر بر روی 100 بیمار در مراکز جراحی اعصاب بیمارستان کاشانی و الزهرا اصفهان در سال 83 -82 انجام شده است. هیپرناترمی (سطح سرمی سدیم بیش از </font><span dir="ltr" style="font-size: 9pt"> 145m mol/L</span><font size="2">) و هیپوناترمی (سطح سرمی سدیم کمتر از </font><span dir="ltr" style="font-size: 9pt">135m mol/L</span><font size="2">) با اندازهگیری سطح سرمی</font><span dir="ltr" style="font-size: 9pt"> </span><font size="2"> سدیم در زمان بستری و روز 3 و 6 و 9 بعد از </font><span dir="ltr" style="font-size: 9pt">SAH</span><font size="2"> تعیین شده و سپس اثرات هیپر و هیپوناترمی بر پیش آگهی 3 ماهه بیماران بعد از تنظیم فاکتورهای متفرقه موثر بر پیش آگهی از قبیل سن، جنس، فشارخون زمینهای، </font><span dir="ltr" style="font-size: 9pt">GCS</span><font size="2"> زمان بستری، میانگین فشار خون شریانی اولیه و یافتههای </font><span dir="ltr" style="font-size: 9pt">CT Scan</span><font size="2"> و آنژیوگرافی مغز ارزیابی شده است. بیماران مبتلا به وازواسپاسم سمپتوماتیک از مطالعه حذف شدند.</font></p>
<p dir="rtl" style="color: rgb(45, 135, 179) font-family: tahoma font-size: 12px font-style: normal font-variant: normal font-weight: normal letter-spacing: normal line-height: 20px text-indent: 0px text-transform: none white-space: normal widows: 1 word-spacing: 0px background-color: rgb(247, 246, 243) text-align: justify"><strong><span style="font-size: 9pt">نتایج:</span></strong><font size="2"> از 100 بیمار مورد بررسی در 18 مورد (18%) هایپرناترمی و 42 مورد (42%) هیپوناترمی وجود داشت هیپرناترمی در مقایسه با هیپوناترمی بطور واضحی با پیش آگهی بد همراه بود (</font><span dir="ltr" style="font-size: 9pt">odds ratio CI 95% 2.63</span><font size="2">).</font></p>
<p style="color: rgb(45, 135, 179) font-family: tahoma font-size: 12px font-style: normal font-variant: normal font-weight: normal letter-spacing: normal line-height: 20px text-indent: 0px text-transform: none white-space: normal widows: 1 word-spacing: 0px background-color: rgb(247, 246, 243) text-align: justify"><strong><span dir="rtl" style="font-size: 9pt">نتیجه نهایی:</span></strong><span dir="rtl" style="font-size: 10pt"> هیپوناترمی شایعتر از هیپرناترمی بعد از SAH می باشد. هیپرناترمی بعد از SAH با پیشآگهی بدبیماری همراه است و این ارتباط غیر وابسته به عواملی از قبیل سن، جنس، GCS بدو ورود و یافتههای CT Scan و آنژیوگرافی مغز میباشد.</span></p>

Hyponatremia and hypernatremia are the most common electrolyte disturbance in acute period of disease, and these disturbances may worsen cerebral edema and mass effect. We performed this study to determine the prognostic significance of serum sodium concentration abnormalities.</span></p>

The presence of hypernatremia (serum sodium concentration of>145m mol/L) and hyponatremia (serum sodium concentration of<135mmol/l) was determined with serum sodium measurements at admission and 3, 6 and 9 days after SAH, then the effects of hypernatremia and hyponatremia on three-month outcomes were analyzed after adjustment for age, sex, preexiting HTN , admission GCS score, initial mean arterial pressure and finding of Brain CT Scan and Brain angiography. Patients with symptomatic vasospasm were excluded.</span></p>
<p style="color: rgb(45, 135, 179) font-family: "Times New Roman" font-size: 16px font-style: normal font-variant: normal font-weight: normal letter-spacing: normal line-height: 22.6667px text-indent: 0px text-transform: none white-space: normal widows: 1 word-spacing: 0px background-color: rgb(247, 246, 243) text-align: justify"><strong><span style="font-size: 10pt">Results:</span><font size="6"> </font></strong><span style="font-size: 12pt">The analysis of data showed that from 100 participated patients in this study 18% developed hypernatremia and 42% developed hyponatremria. Hypernatemia was significantly assiociated with poor outcomes compared with hyponatremia (odds ratio, 2.63- CI 95% ).</span></p>

Hypernatrenia after SAH associted with poor outcome and this association is independet of previously identified outcome predictors, including age, sex, admission GCS Score, and X.Ray finding.</span></p>
