fa ارزیابی حساسیت و ویژگی آزمایش CA19-9 در تشخیص آدنوکارسینوم معده سایر تخصص هاي باليني Other Clinical Specialties پژوهشي Original <p dir="rtl" style="color: rgb(45, 135, 179) font-family: tahoma font-size: 12px font-style: normal font-variant: normal font-weight: normal letter-spacing: normal line-height: 20px text-indent: 0px text-transform: none white-space: normal widows: 1 word-spacing: 0px background-color: rgb(247, 246, 243) text-align: justify"><strong><span style="font-size: 9pt">مقدمه و هدف :</span></strong><font size="2"> آدنوکارسینوم معده یکی از بدخیمی های شایع می باشد و بر اساس آمارهای موجود شیوع آن در ایران پس از سرطان مری در درجه دوم قرار دارد. با توجه به شیوع و شدت این بدخیمی آنچه که بیشتر اهمیت دارد مسأله تشخیص زودرس و جراحی به موقع این تومور است . بهمین منظور این مطالعه با هدف تعیین حساسیت و ویژگی آزمایش <span dir="ltr">CA19-9</span> در تشخیص آدنوکارسینومای معده انجام گرفت.</font></p> <p dir="rtl" style="color: rgb(45, 135, 179) font-family: tahoma font-size: 12px font-style: normal font-variant: normal font-weight: normal letter-spacing: normal line-height: 20px text-indent: 0px text-transform: none white-space: normal widows: 1 word-spacing: 0px background-color: rgb(247, 246, 243) text-align: justify"><strong><span style="font-size: 9pt">روش کار :</span></strong><font size="2"> این پژوهش از نوع مطالعه توصیفی مقطعی می باشد که بصورت آینده نگر انجام شده است. جامعه آماری آنرا 35 بیمار مبتلا به آدنوکارسینوم معده که با پاتولوژی به اثبات رسیده است به عنوان گروه مورد و 35 نفر افراد غیر مبتلا به آدنوکارسینوم معده بعنوان گروه شاهد که به بخش گوارش و پاتولوژی بیمارستان اکباتان شهر همدان مراجعه نموده بودند تشکیل می داد. ابتدا در بیمارانی که با شکایت های گوارشی مشابه از سه ماه قبل مراجعه می کردند آندوسکوپی و بیوپسی انجام می گرفت و بر اساس نتیجه پاتولوژی بیماران در یکی از دو گروه مبتلا به آدنوکارسینوم و گروه شاهد قرار می گرفتند سپس در هر دو گروه تست <span dir="ltr">CA19-9</span> به انجام می رسید و نتایج بدست آمده در چک لیست ثبت می گردید و به کمک نرم افزار <span dir="ltr">EPI6</span> و آزمون <span dir="ltr">&chi</span>^{<span dir="ltr">2</span>} تجزیه و تحلیل می شدند.</font></p> <p dir="rtl" style="color: rgb(45, 135, 179) font-family: tahoma font-size: 12px font-style: normal font-variant: normal font-weight: normal letter-spacing: normal line-height: 20px text-indent: 0px text-transform: none white-space: normal widows: 1 word-spacing: 0px background-color: rgb(247, 246, 243) text-align: justify"><strong><span style="font-size: 9pt">نتایج :</span></strong><font size="2"> این مطالعه نشان داد که نقطه قطع 35 برای <span dir="ltr">CA19-9</span> با حساسیت و ویژگی مطلوب و بالایی برای رد یا تشخیص آدنوکارسینومای معده همراه می باشد بطوریکه در این نقطه حساسیت تست 77.1% و اختصاصیت آن 97.1% می باشد. همچنین نتایج مطالعه نشان داد استفاده از این تست جهت تشخیص آدنوکارسینومای معده قبل از انجام اقدامات تهاجمی بسیار ارزشمند و مفید می باشد.</font></p> <p style="color: rgb(45, 135, 179) font-family: tahoma font-size: 12px font-style: normal font-variant: normal font-weight: normal letter-spacing: normal line-height: 20px text-indent: 0px text-transform: none white-space: normal widows: 1 word-spacing: 0px background-color: rgb(247, 246, 243) text-align: justify"><strong><span dir="rtl" style="font-size: 9pt">نتیجه نهائی :</span></strong><span dir="rtl" style="font-size: 10pt"> تست  CA19-9  دارای حساسیت و ویژگی بالایی جهت تشخیص یا رد آدنوکارسینومای معده می باشد.</span></p> <p style="color: rgb(45, 135, 179) font-family: "Times New Roman" font-size: 16px font-style: normal font-variant: normal font-weight: normal letter-spacing: normal line-height: 22.6667px text-indent: 0px text-transform: none white-space: normal widows: 1 word-spacing: 0px background-color: rgb(247, 246, 243) text-align: justify"><strong><span style="font-size: 10pt">Introduction & Objective:</span></strong><span style="font-size: 12pt"> The objective of this study was to determine the sensitivity and specificity of CA 19-9 test in diagnosis of gastric cancer patients.</span></p> <p style="color: rgb(45, 135, 179) font-family: "Times New Roman" font-size: 16px font-style: normal font-variant: normal font-weight: normal letter-spacing: normal line-height: 22.6667px text-indent: 0px text-transform: none white-space: normal widows: 1 word-spacing: 0px direction: ltr unicode-bidi: embed background-color: rgb(247, 246, 243) text-align: justify"><strong><font size="2">Materials & Methods:</font></strong><span style="font-size: 11pt"> </span><span style="font-size: 12pt">This prospective and cross-sectional study was carried out on 35 patients with gastric cancer and 35 patients without adenocarcinoma of gaster as a control group. Theses patients were referred to alimentary and pathology wards in Ekbatan Hospital of Hamadan city. After selection of the patients who had the inclusion criteria, CA19-9 level was determined, and information collected in a checklist. In cut off point of 35 the sensitivity and specificity was 77.1% and 97.1% respectively.
