fa بررسی ارتباط لپتین با عوامل خطر بیماریهای عروق کرونر قلب در بیماران مبتلا به دیابت نوع II در مقایسه با افراد غیر دیابتی Study of Relationship between Leptin and Risk Factors for Coronary Heart Disease in Patients with Type 2 Diabetes Mellitus and Non Diabetic Controls سایر تخصص هاي باليني Other Clinical Specialties پژوهشي Original <p dir="rtl" style="color: rgb(45, 135, 179) font-family: tahoma font-size: 12px font-style: normal font-variant: normal font-weight: normal letter-spacing: normal line-height: 20px orphans: auto text-indent: 0px text-transform: none white-space: normal widows: 1 word-spacing: 0px -webkit-text-stroke-width: 0px text-align: justify background-color: rgb(247, 246, 243)">مقدمه و هدف : لپتین نقش مهمی در تنظیم وزن بدن و متابولیسم چربی دارد. ارتباط بین لپتین با بافت چربی و نمایه توده بدنی در مطالعات مختلف نشان داده شده است. با توجه به ارتباط بین بیماری های قلبی _ عروقی در حضور دیابت نوع IIو چاقی، محققان را بر آن داشته که به بررسی ارتباط بین لپتین پلاسما با سایر عوامل مستعد کننده بیماری عروق کرونر درغیاب بیماری های عروق کرونر در افراد تحت مطالعه بپردازند. <p dir="rtl" style="color: rgb(45, 135, 179) font-family: tahoma font-size: 12px font-style: normal font-variant: normal font-weight: normal letter-spacing: normal line-height: 20px orphans: auto text-indent: 0px text-transform: none white-space: normal widows: 1 word-spacing: 0px -webkit-text-stroke-width: 0px text-align: justify background-color: rgb(247, 246, 243)">روش کار : مطالعه حاضر یک مطالعه توصیفی ـ مقطعی است که در آن 88 فرد غیردیابتی ( گروه A) با 17 بیمار دیابتی (گروه B) که اسکن پرفیوژن میوکارد طبیعی داشته اند و سابقه بیماریهای کلیوی، کبدی و تیروئیدی را ذکر نمی کردند، از بین 500 بیمار مراجعه کننده به بخش پزشکی هسته ای بیمارستان امام حسین (ع) شاهرود، انتخاب شده اند. بیماران دیابتی شامل موارد شناخته شده مبتلا به دیابت نوع II بوده که تحت درمان با رژیم غذایی یا قرصهای خوراکی پایین آورنده قند خون بودند. بدنبال 12 ساعت ناشتایی، صبح روز مراجعه نمونه خون بیماران گرفته شده و پلاسمای خون بلافاصله جدا شده و سطوح پلاسمایی قند، تری گلیسیرید، LDL و HDL آنها اندازه گیری شده است.لپتین سرم با استفاده از روش رادیو ایمنواسی اندازه گیری شده است. <p dir="rtl" style="color: rgb(45, 135, 179) font-family: tahoma font-size: 12px font-style: normal font-variant: normal font-weight: normal letter-spacing: normal line-height: 20px orphans: auto text-indent: 0px text-transform: none white-space: normal widows: 1 word-spacing: 0px -webkit-text-stroke-width: 0px text-align: justify background-color: rgb(247, 246, 243)">نتایج : در مطالعه حاضر بین میانگین سنی بیماران گروه A و B تفاوت معناداری مشاهده نشد. فراوانی مصرف سیگار، میانگین BMI در گروه B بیشتر از گروه A بود. بین میانگین غلظت لپتین در دو گروه بر حسب جنس نیز تفاوت معناداری مشاهده نشد. از بین متغیرهای مرتبط با بیماری های عروق کرونر فقط BMI با لپتین همبستگی معناداری را نشان داد. <p style="color: rgb(45, 135, 179) font-family: tahoma font-size: 12px font-style: normal font-variant: normal font-weight: normal letter-spacing: normal line-height: 20px orphans: auto text-align: -webkit-right text-indent: 0px text-transform: none white-space: normal widows: 1 word-spacing: 0px -webkit-text-stroke-width: 0px background-color: rgb(247, 246, 243)">نتیجه نهائی : علی رغم رابطه معنادار مشاهده شده بین لپتین و BMI ، لپتین با سایر عوامل خطر بیماری های عروق کرونر از قبیل سن، کلسترول، فشار خون سیستولی و دیاستولی، تری گلیسیرید،HDL و LDL در بیماران دیابتی و غیردیابتی رابطه معناداری را نشان نمی دهد. <p style="color: rgb(45, 135, 179) font-family: 'Times New Roman' font-size: 16px font-style: normal font-variant: normal font-weight: normal letter-spacing: normal line-height: 22.6667px orphans: auto text-indent: 0px text-transform: none white-space: normal widows: 1 word-spacing: 0px -webkit-text-stroke-width: 0px direction: ltr unicode-bidi: embed text-align: justify background-color: rgb(247, 246, 243)">Introduction & Objective: Leptin is considered to play an important role in the regulation of body weight and metabolism. Many studies have shown an association between BMI and leptin concentration. Our study compares the relationship between plasma leptin levels and risk factors for CHD in patients with type 2 diabetes and non-diabetic controls. <p style="color: rgb(45, 135, 179) font-family: 'Times New Roman' font-size: 16px font-style: normal font-variant: normal font-weight: normal letter-spacing: normal line-height: 22.6667px orphans: auto text-indent: 0px text-transform: none white-space: normal widows: 1 word-spacing: 0px -webkit-text-stroke-width: 0px direction: ltr unicode-bidi: embed text-align: justify background-color: rgb(247, 246, 243)">Materials & Methods: A total of 88 patients without diabetes (group A) were compared with 17 diabetic subjects (group B). Patients with CHD according to myocardial perfusion scan results, renal, thyroid and hepatic disorders were excluded from the study. After a 12-hours overnight fast, blood samples were obtained. Plasma was immediately separated for laboratory measurements. Frequency of smoking and BMI mean score were significantly higher in diabetic patients. There were no differences in leptin levels in two groups.
