fa مقایسه بروز مشکلات تنفسی در نوزادان ترم حاصل از سزارین اورژانسی با سزارین انتخابی سایر تخصص هاي باليني Other Clinical Specialties پژوهشي Original <p dir="rtl" style="text-align: justify"><span id="ctl00_ContentPlaceHolder1_DataList1_ctl00_abstractLabel" style="color: rgb(45, 135, 179) font-family: tahoma font-size: 12px font-style: normal font-variant: normal font-weight: normal letter-spacing: normal orphans: auto text-align: -webkit-right text-indent: 0px text-transform: none white-space: normal widows: 1 word-spacing: 0px -webkit-text-stroke-width: 0px line-height: 15pt background-color: rgb(247, 246, 243)"><strong><span style="font-size: 9pt">مقدمه و هدف :</span></strong><font size="2"> اگر چه سزارین های اورژانسی به منظور نجات جان مادر و جنین انجام می گیرد. لیکن موارد انتخابی آن همیشه بدون عارضه نبوده و به ویژه در زایمانهای ترم علا وه بر ایجاد مشکلات و عوارض مادری می تواند مشکلات دوره نوزادی و مرگ و میر مرتبط با آن را افزایش دهد. یکی از بارزترین این مشکلات دیسترس تنفسی بدنبال سزارینهای انتخابی می باشد. این مطالعه با هدف مقایسه میزان بروز مشکلات تنفسی در نوزادان ترم حاصل از سزارین اورژانسی با سزارین انتخابی انجام گرفت.</font></span></p> <p dir="rtl" style="text-align: justify"><span id="ctl00_ContentPlaceHolder1_DataList1_ctl00_abstractLabel" style="color: rgb(45, 135, 179) font-family: tahoma font-size: 12px font-style: normal font-variant: normal font-weight: normal letter-spacing: normal orphans: auto text-align: -webkit-right text-indent: 0px text-transform: none white-space: normal widows: 1 word-spacing: 0px -webkit-text-stroke-width: 0px line-height: 15pt background-color: rgb(247, 246, 243)"><strong><span style="font-size: 9pt">روش کار :</span></strong><font size="2"> در مطالعه حاضر که بصورت کوهورت آینده نگر بر روی 400 مورد زایمان (200 مورد سزارین اورژانسی و200 مورد سزارین انتخابی ) انجام شد ، بروز مشکلات تنفسی و طول بستری نوزادان در دو گروه بررسی و مقایسه شد.</font></span></p> <p dir="rtl" style="text-align: justify"><span id="ctl00_ContentPlaceHolder1_DataList1_ctl00_abstractLabel" style="color: rgb(45, 135, 179) font-family: tahoma font-size: 12px font-style: normal font-variant: normal font-weight: normal letter-spacing: normal orphans: auto text-align: -webkit-right text-indent: 0px text-transform: none white-space: normal widows: 1 word-spacing: 0px -webkit-text-stroke-width: 0px line-height: 15pt background-color: rgb(247, 246, 243)"><strong><span style="font-size: 9pt">نتایج :</span></strong><font size="2"> مطالعه اخیر بروز مشکلات تنفسی را در نوزادان ترم حاصل سزارین های اورژانسی 3.5 درصدودر سزارینهای انتخابی 5/8 درصد نشان داد که اختلاف مشاهده شده در سطح فراتر از5 درصد معنی دار گردید (P<0.05</font><font size="2">). در نوزادان ترم حاصل از سزارینهای اورژانسی طول مدت بستری در 43 درصد موارد 24 ساعت و کمتر و در بقیه موارد بیش از یک روز بود و در نوزادان ترم حاصل از سزارینهای انتخابی در 65 درصد موارد 24 ساعت و کمتر و در 35 درصد بقیه بیش از یک روزبود که اختلاف مشاهده شده در طول مدت بستری از نظر آماری معنی دارنبود (</font><font size="2">P>0.05)</font></span></p> <p style="text-align: justify"><span id="ctl00_ContentPlaceHolder1_DataList1_ctl00_abstractLabel" style="color: rgb(45, 135, 179) font-family: tahoma font-size: 12px font-style: normal font-variant: normal font-weight: normal letter-spacing: normal orphans: auto text-align: -webkit-right text-indent: 0px text-transform: none white-space: normal widows: 1 word-spacing: 0px -webkit-text-stroke-width: 0px line-height: 15pt background-color: rgb(247, 246, 243)"><strong><span dir="rtl" style="font-size: 9pt">نتیجه نهائی :</span></strong><span dir="rtl" style="font-size: 10pt"> بررسی حاضر نشان داد که در گروه سزارین های انتخابی مشکلات تنفسی نوزادان ترم بیشتر است.</span></span></p> <p style="text-align: justify"><span id="ctl00_ContentPlaceHolder1_DataList2_ctl00_abstractLabel0" style="color: rgb(45, 135, 179) font-style: normal font-variant: normal font-weight: normal letter-spacing: normal orphans: auto text-align: -webkit-left text-indent: 0px text-transform: none white-space: normal widows: 1 word-spacing: 0px -webkit-text-stroke-width: 0px font-family: 'Times New Roman' font-size: 12pt line-height: 17pt background-color: rgb(247, 246, 243)"><strong><span style="font-size: 10pt">Introduction & Objective:</span></strong><span style="font-size: 11pt"> Although emergency cesarean section is performed to save mother and neonate life in many cases, but this is not always safe in all cases, especially in elective one. In addition to maternal problems it can increases neonatal morbidity and mortality rate especially in term neonates. One of the most prominent problems in this regard is neonatal respiratory distress following elective cesarean section, which are studied in this research.</span></span></p> <p style="text-align: justify"><span id="ctl00_ContentPlaceHolder1_DataList2_ctl00_abstractLabel0" style="color: rgb(45, 135, 179) font-style: normal font-variant: normal font-weight: normal letter-spacing: normal orphans: auto text-align: -webkit-left text-indent: 0px text-transform: none white-space: normal widows: 1 word-spacing: 0px -webkit-text-stroke-width: 0px font-family: 'Times New Roman' font-size: 12pt line-height: 17pt background-color: rgb(247, 246, 243)"><strong><span style="font-size: 10pt">Materials & Methods :</span></strong><span style="font-size: 11pt"> This was a prospective cohort study. The neonatal respiratory morbidity incidence and admission length between term infants delivered by emergency and elective cesarean section were evaluated and the results were compared between two studied groups(200 cases in each group).</span></span></p> <h3><span id="ctl00_ContentPlaceHolder1_DataList2_ctl00_abstractLabel0" style="color: rgb(45, 135, 179) font-style: normal font-variant: normal font-weight: normal letter-spacing: normal orphans: auto text-align: -webkit-left text-indent: 0px text-transform: none white-space: normal widows: 1 word-spacing: 0px -webkit-text-stroke-width: 0px font-family: 'Times New Roman' font-size: 12pt line-height: 17pt background-color: rgb(247, 246, 243)"><span style="font-size: 10pt">Results:</span><span style="font-weight: normal font-size: 11pt"> Infants delivered by elective cesarean section had an increased rate of respiratory complication compare to infants born by emergency cesarean section (8.5% vs. 3.5%   p<0.05).
