fa بررسی معیارهای درخواست سی تی اسکن مغزی قبل از انجام پونکسیون کمری در کودکان مشکوک به مننژیت در بیمارستان حضرت رسول اکرم(ص) تهران Evaluation of Criteria for Requesting Brain CT Scan before Performing Lumbar Puncture for the Children Suspicious to Meningitis in Rasul Akram Hospital, Tehran سایر تخصص هاي باليني Other Clinical Specialties پژوهشي Original <span id="ctl00_ContentPlaceHolder1_DataList1_ctl00_abstractLabel" style="color: rgb(45, 135, 179); font-family: tahoma; font-size: 12px; font-style: normal; font-variant: normal; font-weight: normal; letter-spacing: normal; orphans: auto; text-align: -webkit-right; text-indent: 0px; text-transform: none; white-space: normal; widows: 1; word-spacing: 0px; -webkit-text-stroke-width: 0px; line-height: 15pt; background-color: rgb(247, 246, 243);">تشخیص مننژیت تنها با انجام پونکسیون کمری Lumbar puncture;LP)) میسر میباشد, ولی همواره برای پزشکان پیش از انجام این اقدام ,نگرانی در مورد عوارض آن وجود داردکه خطر ناک ترین آنها هرنی مغزی میباشد. مشکلات موجود در معاینه عصبی بخصوص در شیرخواران, عدم همکاری کودک و شیرخوار در هنگام انجام کار , باورهای نادرست در سطح جامعه و نگرانی پزشکان از مشکلات قانونی , در اختیار نبودن امکانات انجام سی تی اسکن اورژانس در بسیاری از نقاط کشور,هزینه ها و خطرات مواجهه بی دلیل با اشعه دلایلی هستند که بررسی علل درخواست B.CT (سی تی اسکن مغزی) را پیش از  LPبخصوص در مراکز آموزشی ما که هدف این مطالعه نیز میباشد ضروری مینمایاند. <span id="ctl00_ContentPlaceHolder1_DataList1_ctl00_abstractLabel" style="color: rgb(45, 135, 179); font-family: tahoma; font-size: 12px; font-style: normal; font-variant: normal; font-weight: normal; letter-spacing: normal; orphans: auto; text-align: -webkit-right; text-indent: 0px; text-transform: none; white-space: normal; widows: 1; word-spacing: 0px; -webkit-text-stroke-width: 0px; line-height: 15pt; background-color: rgb(247, 246, 243);">          در این بررسی توصیفی و مقطعی یکساله تعداد 100شیرخوار و کودک دوماهه تا دوازده ساله مشکوک به مننژیت که کاندید انجام LP بودند براساس تشخیص پزشکان مربوطه به دوگروه درخواستB.CT شده و نشده تقسیم شدند و متغیرهای زمینه ای و بالینی احتمالی موثر در این تصمیم گیری با کمک پرسشنامه تکمیل و نتایج B.CT های انجام شده ثبت گردید. یافته های حاصله در دو گروه با کمک آزمون آماری chi-square بررسی شد و P.value<0.05 سطح معنادار بودن تلقی گردید. <span id="ctl00_ContentPlaceHolder1_DataList1_ctl00_abstractLabel" style="color: rgb(45, 135, 179); font-family: tahoma; font-size: 12px; font-style: normal; font-variant: normal; font-weight: normal; letter-spacing: normal; orphans: auto; text-align: -webkit-right; text-indent: 0px; text-transform: none; white-space: normal; widows: 1; word-spacing: 0px; -webkit-text-stroke-width: 0px; line-height: 15pt; background-color: rgb(247, 246, 243);">          پیش از انجام LP در 21 بیمار درخواست B.CT شد (گروه مورد) که نتایج آن تنها در 3 نفر ( 14% ) موارد منجر به کنسل شدن LP گردید. از میان متغیر های مورد بررسی صرفاً داشتن چهره توکسیک , حال عمومی بد, ملاج برجسته و کاهش قابل ملاحظه سطح هشیاری (Glasgow coma scale<8 ) با این تصمیم گیری مرتبط بود . همچنین تمایل به چنین درخواستی با توجه به P value=0.2 با پارامترهایی همچون ایست قلبی تنفسی اخیر, علائم فوکال واضح , ادم پاپی , همی پارزی و برادی کاردی وجود داشت. در طی مطالعه دو مورد هرنی مغزی درگروه شاهد رخ داد که هر دوشیرخوار علائم واضح بیقراری شدید و استفراغ های جهنده داشتند. <span id="ctl00_ContentPlaceHolder1_DataList1_ctl00_abstractLabel" style="color: rgb(45, 135, 179); font-family: tahoma; font-size: 12px; font-style: normal; font-variant: normal; font-weight: normal; letter-spacing: normal; orphans: auto; text-align: -webkit-right; text-indent: 0px; text-transform: none; white-space: normal; widows: 1; word-spacing: 0px; -webkit-text-stroke-width: 0px; line-height: 15pt; background-color: rgb(247, 246, 243);">          کاهش شدید سطح هشیاری وعلائم فوکال در سایر مطالعات نیز جزو معیارهای پذیرفته شده درخواست B.CT پیش از LP  مطرح گردیده اما دو معیار دیگر (حال عمومی و ملاج برجسته) به تنهایی بنظر نمیرسد دلایل منطقی برای چنین درخواستی باشند. مطالعات وسیعتر بخصوص با در نظر گرفتن تشخیصهای افتراقی و محدودیتهای طیف سنی کودکان میتواند به پروتکل های آموزشی مناسب انجامیده و پزشکان را بلحاظ قانونی نیز حمایت نماید. <p style="color: rgb(45, 135, 179); font-family: "Times New Roman"; font-size: 16px; font-style: normal; font-variant: normal; font-weight: normal; letter-spacing: normal; line-height: 22.6667px; text-transform: none; white-space: normal; widows: 1; word-spacing: 0px; direction: ltr; text-indent: 36pt; unicode-bidi: embed; background-color: rgb(247, 246, 243); text-align: justify;">Lumbar puncture (LP) is the only way to diagnose meningitis, however some concerning points regarding its complication especially brain herniation are usually noted. Several factors including difficulties of neurologic examination especially in infants, absence of cooperation when performing the LP, incorrect    myths and legal issues as well as unavailability of CT equipment , X-ray exposure and charges; makes evaluation of such requests reasonable . <p style="color: rgb(45, 135, 179); font-family: "Times New Roman"; font-size: 16px; font-style: normal; font-variant: normal; font-weight: normal; letter-spacing: normal; line-height: 22.6667px; text-transform: none; white-space: normal; widows: 1; word-spacing: 0px; direction: ltr; text-indent: 36pt; unicode-bidi: embed; background-color: rgb(247, 246, 243); text-align: justify;">One hundred infants and children, 2 months to 12 years old who were suspicious to have meningitis were enrolled in this cross-sectional, descriptive study. Two groups were formed based on requesting a B.CT (Brain CT scan) before LP or performing the procedure directly. Demographic and clinical parameters were analyzed among two groups using a questionnaire and CT results were recorded. Chi-square test was used for statistical analysis; P value<0.05 was considered as significant. <p style="color: rgb(45, 135, 179); font-family: "Times New Roman"; font-size: 16px; font-style: normal; font-variant: normal; font-weight: normal; letter-spacing: normal; line-height: 22.6667px; text-transform: none; white-space: normal; widows: 1; word-spacing: 0px; direction: ltr; text-indent: 36pt; unicode-bidi: embed; background-color: rgb(247, 246, 243); text-align: justify;">B.CT was requested for 21 patients (case group) in which the results led to postponing the procedure in 14%( 3) of cases. Toxic appearance, poor general condition, bulging fontanel and impaired consciousness (GCS score<8) were significantly related to such a decision making. There was also a trend toward ordering B.CT for those patients with recent cardiopulmonary arrest, focal signs, papilledema, hemiparesis and bradycardia.
