fa بررسی درگیریهای چشمی در 28 بیمار مبتلا به سندرم بهجت The Study of Ophthalmologic Involvement in 28 Behcet’s Syndrome Patients سایر تخصص هاي باليني Other Clinical Specialties پژوهشي Original <span id="ctl00_ContentPlaceHolder1_DataList1_ctl00_abstractLabel" style="font: 12px/15pt tahoma; color: rgb(45, 135, 179); text-transform: none; text-indent: 0px; letter-spacing: normal; word-spacing: 0px; white-space: normal; widows: 1; font-size-adjust: none; font-stretch: normal; background-color: rgb(247, 246, 243); -webkit-text-stroke-width: 0px;">   بیماری بهجت یک واسکولیت ژنرالیزه با علت ناشناخته می باشد. درگیری چشمی بیماری بهجت بصورت ایریتیس حاد راجعه واسکولیت شبکیه ، خونریزی شبکیه ، ادم ماکولا و . . . می باشد. این مطالعه با هدف تعیین فراوانی درگیریهای چشمی در بیماران مبتلا به بیماری بهجت مراجعه کننده به درمانگاه چشم پزشکی بیمارستان امام خمینی همدان در سال 1381 انجام گرفته است. <span id="ctl00_ContentPlaceHolder1_DataList1_ctl00_abstractLabel" style="font: 12px/15pt tahoma; color: rgb(45, 135, 179); text-transform: none; text-indent: 0px; letter-spacing: normal; word-spacing: 0px; white-space: normal; widows: 1; font-size-adjust: none; font-stretch: normal; background-color: rgb(247, 246, 243); -webkit-text-stroke-width: 0px;">         مطالعه حاضر از نوع توصیفی مقطعی می باشد که به شکل آینده نگر بر روی 28 بیمار مبتلا به سندرم بهجت انجام گرفت. تمامی بیماران در درمانگاه چشم پزشکی مورد معاینه با Slit Lamp ، فوندسکوپی مستقیم و غیر مستقیم و سنجش حدت بینایی قرار گرفتند و اطلاعات بدست آمده در پرسشنامه مخصوص هر بیمار ثبت گردید. داده های جمع آوری شده با استفاده از برنامه نرم افزاری EPI 2000 مورد تجزیه و تحلیل قرار گرفت. <span id="ctl00_ContentPlaceHolder1_DataList1_ctl00_abstractLabel" style="font: 12px/15pt tahoma; color: rgb(45, 135, 179); text-transform: none; text-indent: 0px; letter-spacing: normal; word-spacing: 0px; white-space: normal; widows: 1; font-size-adjust: none; font-stretch: normal; background-color: rgb(247, 246, 243); -webkit-text-stroke-width: 0px;">          از کل بیماران مورد مطالعه 19 نفر مرد و 9 نفر زن بودند. شایع ترین سن بروز بیماری دهه چهارم زندگی بود. درگیری چشمی بصورت کلی در 20 نفر (71%) دیده شد. در 8 نفر بصورت کاتاراکت ، در 3 مورد بصورت پیگمانتاسیون عدسی ، در 2 مورد کاتاراکت کپسول خلفی ، در 11 نفر چسبندگی خلفی عنبیه ، در 2 مورد یووئیت قدامی ، در 3 نفر یوئیت خلفی و در 8 نفر یووئیت منتشر وجود داشت. 3 نفر دارای هایپوپیون بودند ، 11 نفر دارای ویتریتیس و 8 نفر دارای نوروپاتی ایسکمیک رتین ، در 8 نفر نکروز رتین و 8 نفر دارای واسکولیت عروق شبکیه ، 5 نفر دارای خونریزی شبکیه و 5 نفر ادم ماکولا داشتند. درگیری دهانی در 18 نفر (64.3%) اولین علامت بیماری بود. <span id="ctl00_ContentPlaceHolder1_DataList1_ctl00_abstractLabel" style="font: 12px/15pt tahoma; color: rgb(45, 135, 179); text-transform: none; text-indent: 0px; letter-spacing: normal; word-spacing: 0px; white-space: normal; widows: 1; font-size-adjust: none; font-stretch: normal; background-color: rgb(247, 246, 243); -webkit-text-stroke-width: 0px;">          بطور کلی می توان بیان کرد که یووئیت قدامی و کاتاراکت در این مطالعه بیشتر از گزارش سایر مطالعات و یووئیت خلفی و منتشر کمتر از سایر گزارشات بوده است. مابقی نتایج تقریباً با دیگر مطالعات مطابقت دارد. <p style="font: 16px/22.66px "Times New Roman"; text-align: justify; color: rgb(45, 135, 179); text-transform: none; text-indent: 0px; letter-spacing: normal; word-spacing: 0px; white-space: normal; unicode-bidi: embed; direction: ltr; widows: 1; font-size-adjust: none; font-stretch: normal; background-color: rgb(247, 246, 243); -webkit-text-stroke-width: 0px;">Behcet’s syndrome is a generalized vasculitis with unknown cause. Acute recurrent iritis , retinal vasculitis , retinal hemorrhage , macular edema , retinal necrosis , ischemic optic neuropathy and vitritis are the most common ophthalmologic involvement. The aim of this study was to determine the frequency of ophthalmologic involvement in patients with Behcet syndrome who referred to ophthalmology clinic of Imam Khomeini hospital in Hamadan city during May 2002-February 2003. <p style="font: 16px/22.66px "Times New Roman"; text-align: justify; color: rgb(45, 135, 179); text-transform: none; text-indent: 0px; letter-spacing: normal; word-spacing: 0px; white-space: normal; unicode-bidi: embed; direction: ltr; widows: 1; font-size-adjust: none; font-stretch: normal; background-color: rgb(247, 246, 243); -webkit-text-stroke-width: 0px;">          This research was a prospective cross-sectional study on 28 patients suffering from Behcet’s syndrome . All of the patients were examined by slit lamp and direct and indirect fundoscopy and evaluation of visual acuity. The required information from each patiens was recorded in a checklist. <p style="font: 16px/22.66px "Times New Roman"; text-align: justify; color: rgb(45, 135, 179); text-transform: none; text-indent: 0px; letter-spacing: normal; word-spacing: 0px; white-space: normal; unicode-bidi: embed; direction: ltr; widows: 1; font-size-adjust: none; font-stretch: normal; background-color: rgb(247, 246, 243); -webkit-text-stroke-width: 0px;">          19 patients were male and 9 patients were female. The most common age of the patients was the 4th decade. The first sign was oral involvement (64.3%). Ophthalmologic involvement was observed in 20 patients (71%) overall.  Cataract was in 8 patients , pigmentation of lens was in 3 patients and post subcapsular cataract in 2 patients, post synechia was in 11 patients , Ant.uveitis was in 2 patints, post uveitis was in 3 patients and panuveitis was in 8 patients , 3 patients had hypopyone , 11 patients had vitritis , 8 patients had ischemic optic neuropathy , 8 patients had retinal    necrosis , 8 patients had retinal vasculitis , 5 patients had retinal hemorrhage and 5 patients had macular edema.
