fa بررسی فراوانی درماتیت کهنه در کودکان زیر 2 سال مهد کودکهای شهر همدان طی سالهای 81-1380 The Frequency of Diaper Dermatitis in 0-2 Years Old Children at Nursing Homes in Hamadan on 2000-2001 سایر تخصص هاي باليني Other Clinical Specialties پژوهشي Original <span id="ctl00_ContentPlaceHolder1_DataList1_ctl00_abstractLabel" style="font: 12px/15pt tahoma; color: rgb(45, 135, 179); text-transform: none; text-indent: 0px; letter-spacing: normal; word-spacing: 0px; white-space: normal; widows: 1; font-size-adjust: none; font-stretch: normal; background-color: rgb(247, 246, 243); -webkit-text-stroke-width: 0px;">شایعترین بیماری التهابی در کودکان درماتیت کهنه ( دیاپر درماتیت ) می باشد. هرچند علت دقیق بیماری مشخص نیست ولی عواملی مانند اصطکاک مکانیکی، رطوبت، تماس با ادرار و مدفوع، تغییرات pH پوست را در پاتوژنز آن دخیل می دانند. به دلیل اینکه در اغلب موارد درمان بیماری در منزل صورت می گیرد برآورد دقیق فراوانی آن مشکل است ولی فراوانی بیماری از 7% تا 35% و حتی تا 50% گزارش شده است. هدف اصلی مطالعه حاضر تعیین فراوانی درماتیت کهنه و بررسی نقش عوامل دخیل در ایجاد آن در میان کودکان زیر دو سال مهد کودکهای شهر همدان طی سالهای 81-1380 بوده است. <span id="ctl00_ContentPlaceHolder1_DataList1_ctl00_abstractLabel" style="font: 12px/15pt tahoma; color: rgb(45, 135, 179); text-transform: none; text-indent: 0px; letter-spacing: normal; word-spacing: 0px; white-space: normal; widows: 1; font-size-adjust: none; font-stretch: normal; background-color: rgb(247, 246, 243); -webkit-text-stroke-width: 0px;">          در این پژوهش مقطعی- توصیفی با مراجعه به مهد کودکهای شهر همدان حجم نمونه ای معادل 385 کودک زیر دو سال بطور تصادفی انتخاب و مورد بررسی قرار گرفتند. <span id="ctl00_ContentPlaceHolder1_DataList1_ctl00_abstractLabel" style="font: 12px/15pt tahoma; color: rgb(45, 135, 179); text-transform: none; text-indent: 0px; letter-spacing: normal; word-spacing: 0px; white-space: normal; widows: 1; font-size-adjust: none; font-stretch: normal; background-color: rgb(247, 246, 243); -webkit-text-stroke-width: 0px;">          از میان کودکان مورد مطالعه 79 نفر (20.5%) مبتلا به درماتیت کهنه بودند. رابطه بین شیوع بیماری با سطح تحصیلات مادران ، رده سنی کودک و استفاده از پوشک یکبار مصرف معنی دار بود (P<0.05 ). شایعترین ناحیه ابتلا، درگیری توام ژنیتالیا، پرینه ، باتوک و ران بود. رابطه معنی داری بین فراوانی بیماری و جنسیت بیمار مشاهده نشد. <span id="ctl00_ContentPlaceHolder1_DataList1_ctl00_abstractLabel" style="font: 12px/15pt tahoma; color: rgb(45, 135, 179); text-transform: none; text-indent: 0px; letter-spacing: normal; word-spacing: 0px; white-space: normal; widows: 1; font-size-adjust: none; font-stretch: normal; background-color: rgb(247, 246, 243); -webkit-text-stroke-width: 0px;">          به منظور کنترل بیماری یافته های مطالعه بر آموزش بهداشت کودک برای مادران و مربیان مهدهای کودک و استفاده از پوشکهای با قدرت جذب بالای آب تاکید می&rlm;کند. <span id="ctl00_ContentPlaceHolder1_DataList2_ctl00_abstractLabel0" style="font: 12pt/17pt "Times New Roman"; color: rgb(45, 135, 179); text-transform: none; text-indent: 0px; letter-spacing: normal; word-spacing: 0px; white-space: normal; widows: 1; font-size-adjust: none; font-stretch: normal; background-color: rgb(247, 246, 243); -webkit-text-stroke-width: 0px;">Diaper dermatitis(Diaper rash) is the most common cutaneous disorder of infancy and childhood. Although the etiology of this condition remains unknown, an amalgam of various causes (mechanical friction, moisture, contact with urine and feces, variation of skin pH, fungal infection) bring about this common dermatitis. Because most diaper dermatitis is treated conservatively at home, the frequency is unknown, but from 7%-35%    up to 50% has been reported. Therefore the frequency of diaper dermatitis and related aggravating factors in 1-2 years old children was studied at nursing home of Hamadan at year 2000-2001. <span id="ctl00_ContentPlaceHolder1_DataList2_ctl00_abstractLabel0" style="font: 12pt/17pt "Times New Roman"; color: rgb(45, 135, 179); text-transform: none; text-indent: 0px; letter-spacing: normal; word-spacing: 0px; white-space: normal; widows: 1; font-size-adjust: none; font-stretch: normal; background-color: rgb(247, 246, 243); -webkit-text-stroke-width: 0px;">          The study was cross-sectional, descriptive on 385 children usingrandomized selection and data was collected by questionnaire and completed by direct observation. <span id="ctl00_ContentPlaceHolder1_DataList2_ctl00_abstractLabel0" style="font: 12pt/17pt "Times New Roman"; color: rgb(45, 135, 179); text-transform: none; text-indent: 0px; letter-spacing: normal; word-spacing: 0px; white-space: normal; widows: 1; font-size-adjust: none; font-stretch: normal; background-color: rgb(247, 246, 243); -webkit-text-stroke-width: 0px;">          In this study 79 (20.5%) subjects had diaper dermatitis, there was meaningful association with” educational level of mother, age of child and use of disposable diaper(p<0.05)” . There was no meaningful relation with sex.
