fa فراوانی محدودیت حرکتی دردناک شانه بدنبال گچ گیری بلند اندام فوقانی Frequency of Painful Shoulder Limitation of Motion after Long Casting of Upper Extremity سایر تخصص هاي باليني Other Clinical Specialties پژوهشي Original <p dir="rtl" style="font: 12px/20px tahoma; text-align: justify; color: rgb(45, 135, 179); text-transform: none; text-indent: 0px; letter-spacing: normal; word-spacing: 0px; white-space: normal; widows: 1; font-size-adjust: none; font-stretch: normal; background-color: rgb(247, 246, 243); -webkit-text-stroke-width: 0px;">  محدودیت حرکتی دردناک مفصل شانه عارضه شایع و تا حدودی ناشناخته مفصل گلنوهومرال است که از خصوصیات آن درد و محدودیت حرکتی در تمام جهات می باشد. به منظور تعیین فراوانی این بیماری بدنبال گچ گیری بلند اندام فوقانی و ارتباط آن با سن ، جنس و میزان سواد این مطالعه بر روی گروههای مختلف سنی بیماران انجام گرفت. <p dir="rtl" style="font: 12px/20px tahoma; text-align: justify; color: rgb(45, 135, 179); text-transform: none; text-indent: 0px; letter-spacing: normal; word-spacing: 0px; white-space: normal; widows: 1; font-size-adjust: none; font-stretch: normal; background-color: rgb(247, 246, 243); -webkit-text-stroke-width: 0px;">          این پژوهش از نوع مطالعات توصیفی تحلیلی می باشد که بصورت آینده نگر بر روی 388 نفر از بیماران مراجعه کننده به بخش ارتوپدی بیمارستان شهید مباشر کاشانی همدان در طی سال 1380 انجام گرفت. کلیه بیماران به 10 گروه سنی تقسیم بندی شدند و در سه نوبت1 ، 1.5 و 3 ماه پس از آغاز گچ گیری تحت معاینه قرار گرفتند ، اطلاعات بدست آمده پس از ثبت در چک لیست و پرسشنامه به کمک نرم افزار EPI6 و با آزمون آماری χ2 و Anova تست گردیدند. <p dir="rtl" style="font: 12px/20px tahoma; text-align: justify; color: rgb(45, 135, 179); text-transform: none; text-indent: 0px; letter-spacing: normal; word-spacing: 0px; white-space: normal; widows: 1; font-size-adjust: none; font-stretch: normal; background-color: rgb(247, 246, 243); -webkit-text-stroke-width: 0px;">          از لحاظ محدودیت حرکتی در باز شدن شانه در 3 نوبت معاینه پس از درمان بین 10 گروه سنی بیماران تفاوت معنی داری مشاهده شد (P<0.05). محدودیت حرکتی در چرخش به داخل شانه در پایان 3 ماه پس از درمان در 74 نفر (19.1%) دیده شد که از نظر آماری در ماههای مختلف تفاوت معنی داری وجود داشت (P=0.0001). محدودیت حرکتی نهائی در 59.5% زنان و 40.5% مردان وجود داشت که نشان دهنده ارتباط معنی داری است (P=0.001). از کل بیماران مورد مطالعه 73.5% پس از گذشت 3 ماه محدودیت حرکتی نداشتند و 26.5% دارای محدودیت حرکتی نهائی بودند که بیشتر در رده سنی 80-50 سال قرار داشتند. محدودیت حرکتی در سه مقطع تحصیلی در پایان ماه سوم پس از درمان به ترتیب 54.1% در بی سوادان ، 24.5% زیر دیپلم و 21.4% در افراد دیپلم و بالاتر دیده شد. <p style="font: 12px/20px tahoma; text-align: justify; color: rgb(45, 135, 179); text-transform: none; text-indent: 0px; letter-spacing: normal; word-spacing: 0px; white-space: normal; widows: 1; font-size-adjust: none; font-stretch: normal; background-color: rgb(247, 246, 243); -webkit-text-stroke-width: 0px;">          نتیجه نهائی اینکه محدودیت حرکتی دردناک شانه در محدوده سنی 50 تا 80 سال از فراوانی بیشتری برخوردار است ، فراوانی آن در جنس مؤنث بیشتر از مردان دیده می شود و افراد بیسواد از محدودیت حرکتی بیشتری برخوردارند. <span id="ctl00_ContentPlaceHolder1_DataList2_ctl00_abstractLabel0" style="font: 12pt/17pt "Times New Roman"; color: rgb(45, 135, 179); text-transform: none; text-indent: 0px; letter-spacing: normal; word-spacing: 0px; white-space: normal; widows: 1; font-size-adjust: none; font-stretch: normal; background-color: rgb(247, 246, 243); -webkit-text-stroke-width: 0px;">The aim of this study was determination of frequency of painful reduced shoulder motion after long casting of upper extremity and its relation with age , sex and education . <span id="ctl00_ContentPlaceHolder1_DataList2_ctl00_abstractLabel0" style="font: 12pt/17pt "Times New Roman"; color: rgb(45, 135, 179); text-transform: none; text-indent: 0px; letter-spacing: normal; word-spacing: 0px; white-space: normal; widows: 1; font-size-adjust: none; font-stretch: normal; background-color: rgb(247, 246, 243); -webkit-text-stroke-width: 0px;">           The present work was a descriptive analytic prospective study and included 388 patients who referred to Mobasher hospital of Hamadan during 2001. The selected patients in recurrent referring to orthopedic department were classified to 10 age groups and were examined by researcher in 1 , 1.5 and 3 months after treatment and data was collected in check list. The primary data were analyzed with c2 & Anova by employing    EPI 6. <span id="ctl00_ContentPlaceHolder1_DataList2_ctl00_abstractLabel0" style="font: 12pt/17pt "Times New Roman"; color: rgb(45, 135, 179); text-transform: none; text-indent: 0px; letter-spacing: normal; word-spacing: 0px; white-space: normal; widows: 1; font-size-adjust: none; font-stretch: normal; background-color: rgb(247, 246, 243); -webkit-text-stroke-width: 0px;">          Out of 388 studied patients 73.5% after 3 months had no mobility limitation and 26.5% had some limitation. There was significant statistical difference in limitation of abduction shoulder joint movement after 1 , 1.5 and 3 months after treatment among 10 different age groups (P<0.05). Mobility limitation of internal rotation after 3 months in 74 cases (19.1%)(P=0.0001). Final mobility limitation in 59.5% of female patients and 40.5% of male patients(P=0.001).
