fa گزارش یک مورد ساکوئیدوز با تظاهر آدنوپاتی زیر چانه ای و نارسایی کلیه A Case Report of Sarcoidosis with Presentation of Submental Adenopathy and Renal Failure سایر تخصص هاي باليني Other Clinical Specialties گزارش موردي Case Report <span id="ctl00_ContentPlaceHolder1_DataList1_ctl00_abstractLabel" style="font: 12px/15pt tahoma; color: rgb(45, 135, 179); text-transform: none; text-indent: 0px; letter-spacing: normal; word-spacing: 0px; white-space: normal; widows: 1; font-size-adjust: none; font-stretch: normal; background-color: rgb(247, 246, 243); -webkit-text-stroke-width: 0px;">در این گزارش شرح حال زن 68 ساله‌ای بیان می شود که بعلت بی اشتهایی به درمانگاه مرکز آموزشی درمانی هاجر شهرکرد مراجعه نموده بود. <span id="ctl00_ContentPlaceHolder1_DataList1_ctl00_abstractLabel" style="font: 12px/15pt tahoma; color: rgb(45, 135, 179); text-transform: none; text-indent: 0px; letter-spacing: normal; word-spacing: 0px; white-space: normal; widows: 1; font-size-adjust: none; font-stretch: normal; background-color: rgb(247, 246, 243); -webkit-text-stroke-width: 0px;">          بی اشتهایی از حدود چهار ماه پیش از مراجعه آغاز شده و همراه با سیری زودرس، تهوع، یبوست شدید، پرنوشی و Nocturia بوده است. همچنین بیمار طی 4 ماه  10 کیلوگرم کاهش وزن داشته است در طی معاینات فشارخون mmHg         و سه عدد لنفادنوپاتی بااندازه های 1×1 سانتیمتر در زیر چانه داشت و طحال Palpable بوده، سایر معاینات نرمال بودند نتایج بررسی های انجام شده قبلی بیمار که با شک به بدخیمی انجام شده بود شامل رادیوگرافی مری، معده و دوازده، آندوسکوپی دستگاه گوارش فوقانینرمال. سونوگرافی و سی تی اسکن شکم اسپلنومگالی mild ، آزمونهای انجام شده شامل: mg/dl     BUN=40 mg/dl, Creat=2.9 mg/dl, Ca=12.5 و بقیه موارد نرمال بود. <span id="ctl00_ContentPlaceHolder1_DataList1_ctl00_abstractLabel" style="font: 12px/15pt tahoma; color: rgb(45, 135, 179); text-transform: none; text-indent: 0px; letter-spacing: normal; word-spacing: 0px; white-space: normal; widows: 1; font-size-adjust: none; font-stretch: normal; background-color: rgb(247, 246, 243); -webkit-text-stroke-width: 0px;">          در زمان بستر‌ی در ادامه بررسی ها با احتمال وجود بدخیمی شامل لنفوم و یا تومر پستان و یا میلوم مولتیپل بررسی ها را ادامه داده که با توجه به نرسیدن به نتیجه، اقدام به بیوپسی از غده لنفی زیر چانه ای کردیم که نتیجه سارکوئیدوز گزارش شده سطح ACE  سرمی بالا بود. پس از درمان با کورتن، حال بیمار روبه بهبودی رفت و کراتین بیمار رو به کاهش نهاد، آخرین کراتینین Creat=1/2 mg/dl  بود. <span id="ctl00_ContentPlaceHolder1_DataList2_ctl00_abstractLabel0" style="font: 12pt/17pt "Times New Roman"; color: rgb(45, 135, 179); text-transform: none; text-indent: 0px; letter-spacing: normal; word-spacing: 0px; white-space: normal; widows: 1; font-size-adjust: none; font-stretch: normal; background-color: rgb(247, 246, 243); -webkit-text-stroke-width: 0px;">  A 68 years old women refer to our clinical of educational hospital    because of decrease appetite from 4 months ago , with constipation, polyuria and polydypsia, also she had 10 kg weigh loss during this time. In  examination: she had blood pressure of       mmHg, 3 small lymph nodes in submental region (1×1cm) also palpable  spleen. other examinations was totally normal. Befor admission she had evaluated for probable malignany by the resean of her syrptoms, The results y evaluation was: upper Gi-series ,    upper Gi endo scopy were normal . <span id="ctl00_ContentPlaceHolder1_DataList2_ctl00_abstractLabel0" style="font: 12pt/17pt "Times New Roman"; color: rgb(45, 135, 179); text-transform: none; text-indent: 0px; letter-spacing: normal; word-spacing: 0px; white-space: normal; widows: 1; font-size-adjust: none; font-stretch: normal; background-color: rgb(247, 246, 243); -webkit-text-stroke-width: 0px;">          Abdominal sonograply and CT scan with contrast agents show only mild splenomegaly. Creat= 2.9 mg/dl. BUN=40 mg/dl ca=12.5 mg/dl p=3/06 mg/dl Alp=185(100-240), CFR »35 cc/min . PTH was in normal limit. We evaluated the patient for probable solid tumors of breast and multiple myeloma. The results of evaluation were normal, thus we refer the patient for lymph node biopsy (submental lymphadenopathy).
