fa بررسی میانگین زمان سپری شده در بیماران دچار ضربه سر از زمان حادثه تا انتقال بیمار به بیمارستان مباشر کاشانی همدان و شروع اقدامات تشخیصی درمانی در سال 1379 سایر تخصص هاي باليني Other Clinical Specialties پژوهشي Original <p dir="RTL" style="text-align: justify;">&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; سوانح و حوادث از علل مهم مرگ و میر در جوامع امروزی بوده و تقریباً در نیمی از موارد مرگ و میر به دنبال تروما ، ضربه به سر دخیل می باشد. در درمان بیماران دچار ضربه به سر زمان سپری شده از زمان حادثه تا شروع اقدامات در پیش آگهی مصدومین نقش مهمی دارد. این مطالعه با هدف&nbsp; تعیین میانگین زمان سپری شده در بیماران دچار ضربه سر از زمان حادثه تا انتقال بیمار به بیمارستان مباشر کاشانی همدان و شروع اقدامات تشخیصی و درمانی در ماههای تیر و مرداد سال 1379 انجام شد.</p> <p dir="RTL" style="text-align: justify;">&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; این پژوهش از نوع توصیفی تحلیلی است وبرای تمام بیماران بستری شده طی مدت مذکور اطلاعات از طریق مصاحبه با همراهان ، خود بیمار ، پرسنل فوریتهای پزشکی وبررسی برگه های اعزام جمع آوری شده است . در این مطالعه تعداد 140 بیمار با ترومای سر مورد بررسی قرار گرفت.</p> <p dir="RTL" style="text-align: justify;">&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; میانگین زمان انتقال بدست آمده در بیماران غیر اعزامی 3/92<span dir="LTR">&plusmn;</span>2/101 دقیقه، در بیماران اعزامی 1/105<span dir="LTR">&plusmn;</span>221 دقیقه و در مجموع بیماران 7/98<span dir="LTR">&plusmn;</span>139 دقیقه بود&nbsp; میانگین زمان از بدو ورود بیمار تا شروع اقدامات درمانی 1/61<span dir="LTR">&plusmn;</span>8/76 دقیقه و تا انجام سی تی اسکن مغزی 1/120<span dir="LTR">&plusmn;</span>6/150 دقیقه بوده است.شایعترین علل تاخیر در انتقال بیماران ، دوری راه و نبودن وسیله نقلیه بوده است . بیشترین علت تاخیر در انجام سی تی اسکن مغزی ، تاخیر در فرستادن بیمار جهت انجام سی تی اسکن توسط پرسنل بوده است.</p> <p dir="RTL" style="text-align: justify;">&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; میانگین زمان انتقال مصدومین ضربه مغزی در منطقه&nbsp; همدان خصوصا&quot; بیماران اعزام شده از سایر شهرستانها طولانی تر از استانداردهای جهانی است ورفع این معضل مستلزم اقدامات پرسنل بخش های اورژانس و درمانی ونیزسایر بخش های توسعه چون راهنمایی و رانندگی ، وزارت راه ، . . . می باشد .</p> <div> <p style="text-align: justify;">Accident and crashes are major causes of mortality in today&rsquo;s society. In<br> &nbsp;&nbsp; approximately one half of the cases of death are following trauma and head<br> &nbsp;&nbsp; injuries. In cure and recovery of patients , the elapsed time from the accident till<br> &nbsp;&nbsp; the arrival of the patient and the start of medical procedures has a significant role.<br> &nbsp;&nbsp; This survey has been conducted in order to determine the average elapsed time in<br> &nbsp;&nbsp; relation to head injury patients from the time of the accident until arrival at the<br> &nbsp;&nbsp; Mobasher Kashany hospital of Hamadan and beginning of diagnostic and<br> &nbsp;&nbsp; therapeutic treatment.</p> <p style="text-align: justify;">&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; This descriptive research project was conducted from the twenty fourth of June<br> &nbsp;&nbsp; until the twenty second of August in the year 2000. All of the information for this<br> &nbsp;&nbsp; survey was obtained from the patients, their companions, emergency room<br> &nbsp;&nbsp; personnel and study of admission forms. One hundred and forty head trauma<br> &nbsp;&nbsp; patients were assayed.</p> <p style="text-align: justify;">&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; Results showed that the average transportation time obtained in non referral<br> &nbsp;&nbsp; patients was 101.2&plusmn;92.3 minutes , and in referral patients it was 221&plusmn;105.1 minutes.<br> &nbsp;&nbsp; The average for all one hundred and forty patients was 139&plusmn;98.7 minutes. The<br> &nbsp;&nbsp; average time from arrival till the start of thrapeutic action was 76.8&plusmn;61.1 minutes ,<br> &nbsp;&nbsp; and until performance of the brain CT scan was 150&plusmn;120.1 minutes. The most<br> &nbsp;&nbsp; prevalence causes of transportation delays were the long distances to be travelled<br> &nbsp;&nbsp; and non availability of appropriate vehicles for transportation.
