Background and Objective: Tinnitus in postmenopausal women may worsen due to hormonal changes. This study aimed to determine the effect of hormone replacement therapy (HRT) on tinnitus in postmenopausal women.
Materials and Methods: This study was a triple-blind randomized clinical trial conducted on 35 postmenopausal women with tinnitus. The intervention group consisted of 18 patients who received 0.625 mg of conjugated estrogens and 2.5 mg of medroxyprogesterone daily for three months, and the control group consisted of 17 patients who received placebo. Hearing assessments, Tinnitus Handicap Inventory (THI) questionnaire, Visual Analog Scale (VAS), and Tinnitus Matching Test (TMT) were used. SPSS (version 26) software and Mann-Whitney, Wilcoxon, and Spearman correlation tests were used at a significance level of 0.05.
Results: Before treatment, there was no difference between the two groups. After treatment, tinnitus loudness scores in both groups improved on the THI, TMT, and VAS (P < 0.05). In the intervention group, mean tinnitus loudness scores based on THI (P = 0.02) and TMT (P = 0.00) improved. The difference in mean tinnitus loudness scores before and after treatment, compared to controls, was significant only for THI (P = 0.00). The only significant correlation observed in the intervention group was between tinnitus duration and THI (P = 0.03).
Conclusion: In postmenopausal women, HRT may have potential benefits in reducing tinnitus-related disability. While other measures of tinnitus (visual analog scale, tinnitus matching test) are inconsistent or not significant, we cannot conclude that HRT is an effective intervention for tinnitus.
