Background and Objective: Gallbladder cancer (GBC) is a rare but highly fatal biliary tract malignancy with a poor prognosis due to late diagnosis. This study aimed to examine temporal trends and the geographical distribution of age-standardized incidence, mortality, and disability-adjusted life years (DALYs) attributable to GBC in Iran from 1990 to 2023.
Materials and Methods: In this ecological study, age-standardized incidence, mortality, and DALY rates for GBC among Iranian adults aged 20 years were obtained from the Global Burden of Disease study. Temporal trends were assessed using Joinpoint regression analysis to estimate annual percent change and average annual percent change. Additionally, the geographic distribution of the indicators across the country's provinces was mapped in 2023.
Results: Between 1990 and 2023, the age-standardized incidence rate declined by an average of 1.04% annually, while mortality and DALY rates decreased by 1.03% and 1.22% per year, respectively. The declining trends were more pronounced in women than in men. In 2023, the provinces of Qom, Zanjan, Kurdistan, Lorestan, and East Azerbaijan exhibited the highest age-standardized incidence, mortality, and DALY rates, whereas Fars, Hormozgan, Bushehr, and Gilan had the lowest rates.
Conclusion: Although the burden of gallbladder cancer in Iran has declined over the past three decades, substantial temporal fluctuations and geographical disparities remain. These findings highlight the need to strengthen the national cancer registry, improve access to diagnostic and treatment services, and implement targeted preventive interventions in high-risk provinces.
